骨转移瘤核素显像与MR弥散加权成像结果的对比研究 被引量:4

A Comparative Study between Radionuclide Bone Scan and MR Diffusion Weighted Imaging in the Detection of Malignant Bone Metastases

摘要 目的比较放射性核素骨显像与磁共振背景抑制全身弥散加权成像(MR WB-DWIBS)对骨转移瘤的诊断效能。方法 35例经病理证实的恶性肿瘤患者,均于2周内先后行放射性核素骨显像和WB-DWIBS,对两种检查结果进行分析,比较两者对骨转移瘤诊断的灵敏度及特异性。结果两种检查方法共检出病灶106个,最终经临床确诊骨转移灶64个,其中核素骨显像检出转移灶61个,WB-DWIBS共检出转移灶56个。放射性核素骨显像和WB-DWIBS诊断骨转移瘤的灵敏度分别为95.3%和87.5%,差异无统计学意义(P>0.05);特异性分别为71.4%和90.5%,差异有统计学意义(P<0.05)。结论放射性核素骨显像与磁共振WB-DWIBS成像诊断骨转移瘤各有特点,联合应用可提高诊断正确性。 Objective To compare the diagnostic efficacy of radionuclide bone scan (BS)with Whole Body MR diffusion weighted imaging with background suppression (MR WB-DWlBS) for the detection of malignant bone metastases. Methods Totally 35 patients with malignant tumors were enrolled. All the cases underwent BS and WB-DWIBS within two weeks. The re- sults of the above-mentioned examinations were compared, and the sensitivity and specificity between them were analyzed with chi-square test, Results A total of 106 abnormal foci were i- dentified from BS and/or WB-DWIBS, and 64 of them were diagnosed as foci of bone metas- tases. 61 bone metastatic lesions were detected by BS and 56 by DWlBS. The diagnostic sen- sitivity of BS and WB-DWlBS were 95.3% and 87.5% (P〉0.05), while the specificity was 71.4% and 90.5% (P〈0.05), respectively. Cenclusien Radionuclide bone scan and MR WB- DWlBS have their respective advantages in the detection of malignant bone metastases. Com- bination of radionuclide bone scan and MR WB-DWlBS will improve the diagnostic accuracy in bone metastasis detection.
