频域光学相干断层扫描在急性Vogt-小柳-原田综合征临床诊治的意义 被引量:1

The Value of Spectral-domain OCT in the Process of Diagnosis and Treatment in Vogt-Koyanagi-Harada Syndrome

摘要 目的:分析在VKH临床病例的诊治过程中,频域OCT的意义。方法:对2009年1月~2011年8月在我院确诊、治疗和随诊的26例(52眼)VKH患者的临床资料进行回顾性分析,所有患者均予以糖皮质激素规范性治疗,在治疗前后不同时间点进行了B超、FFA、OCT等检查。结果:所有患者经及时诊断和大剂量糖皮质激素规范性治疗,炎症均得到了一定程度的控制,视力明显提高。治疗2周后,52只眼中59.6%以上远视力恢复至≥0.5,B超示玻璃体混浊及脉络膜增厚明显减轻,FFA示视网膜渗漏明显减轻,OCT示视网膜神经上皮层及色素上皮层的脱离高度明显下降,视盘周围RNFL厚度明显下降接近正常。结论:OCT为非创伤性检查,操作简单,快捷,可重复使用,通过OCT检查可以减少VKH的误诊率,促进早期正确诊断,可以直观的了解VKH疾病的视网膜脉络膜组织学改变,可以量化分析病情的转归,指导临床用药,因而频域OCT检查对急性Vogt-小柳-原田综合征的临床诊治有着重要的意义。 Objective: To analyze the value of spectral-domain OCT in the process of diagnosis and treatment in VKH. Methods: Retrospectively analyzed 26 patients (52 eyes) of VKH patients treatment and follow-up in our hospital since January 2009 to August 2011. All patients were received glucocorticoid normative treatment, and accepted B scan, FFA, OCT etc check before and after treatment. Results: After the prompt diagnosis and high-dose glucocorticoids normative treatment, All the patients had a certain degree of control of inflammation and the visual acuity increased obviously. After 2 weeks treatment, 52 eyes (59.6%) recovered to 0.5 in distant vision check, B ultrasonic and vitreous opacity and choroid thickening significantly reduce, FFA shows retinal leakage had be significantly reduce, OCT shows retinal neurepithelium layer and pigment epithelium height decreases obviously, optic disk surrounding RNFL thickness decreased close to normal. Conclusions: OCT is a kind of instrument examination that are noninvasive, easy to operation, check quickly and repeatable. Application of OCT examination can reduce the misdiagnosis rate of VKH, promote accurate diagnosis rate in early phase and intuitive understanding of retinal choroid histology change of VKH disease, and help to analyze quantitatively the outcome and guide the clinical medication. So we can say spectral-domain OCT examination has important significance in clinical diagnosis and treatment of acute Vogt - Koyanagi - harada syndrome.
