摘要
目的观察米非司酮联合米索前列醇清宫术治疗胚胎停止发育的疗效。方法将胚胎停止发育的患者96例随机分为两组,治疗组(53例)给予米非司酮和米索前列醇后行清宫术;对照组(43例)单纯给予米索前列醇后行清宫术。观察两组患者手术时间、术中出血量、宫颈软化及扩张程度、胚胎自然排出率及一次清宫成功率。结果治疗组宫颈软化及扩张的总有效率为94.3%,显著高于对照组53.5%,差异有统计学意义(P<0.05);与对照组比较,治疗组手术时间、术中出血量显著减少,胚胎自然排出率升高,差异均有统计学意义(P<0.05)。结论米非司酮联合米索前列醇清宫术可降低手术难度,减少术中出血及术后并发症,是治疗胎停育一种安全有效的方法。
Objective To explore clinic effect of mifepristone and misoprostol combined with complete curettage of uterine cavity in the treatment of embryonic arrest. Methods 96 cases of embryonic arrest patients were randomly divided into two groups. The treatment group （n=53）was performed by complete curettage of uterine cavity after given mifepristone and misoprostol,and control group （n=43） was performed by complete curettage of uterine cavity after given misoprostol only. The operation time,bleed loss,degree of softening and extension of cervix,the rate of embryonic effluence and achievement ratio of cleaing uterus with one time were observed and analyzed. Results The effective rate of softening and extension of cervix in treatment group（94.3%） was significantly higher than those in control group （53.5%）（P 0.05）.There was significant difference between the two groups in the operation time,bleed loss,incidence of induced abortion syndrome and the rate of embryonic effluence（P 0.05）. Conclusion It is an effective and safe method of mifepristone and misoprostol combined with complete curettage of uterine cavity in the treatment of embryonic arrest,it can contribute to reduce the operation difficulty,bleed loss and the incidence of complications.
