摘要
目的探讨产科人性化护理在提高自然分娩率,促进母婴健康、降低并发症等方面的作用。方法收集2006年1月至2008年12月在普通方法护理下进行分娩的720例单胎头位,无明显剖宫产指征的初产妇为对照组;以2009年1月至2011年12月实行人性化护理,单胎头位、无明显剖宫产指征的初产妇1020例为观察组。比较两组经阴道分娩率、产后出血率、剖宫产率、胎儿窘迫、新生儿窒息发生率。结果观察组自然分娩率高于对照组(89.90% vs64.17%,χ2=170.010,P〈0.05),产后出血率(1.71% vs 3.27%,χ2=5.096,P〈0.05)、剖宫产率(10.10% vs 35.83%,χ2=170.010,P〈0.05)、胎儿窘迫(3.08%vs 4.92%,χ2=19.010,P〈0.05)及新生儿窒息率(1.08% vs 3.06%,χ2=8.868,P〈0.05)均低于对照组。结论产科人性化护理服务模式,对绝大部分产妇适用,对保护、支持自然分娩、提高产时服务质量、保证母婴健康将有重要意义,值得推广。
Objective To discuss the effects of humanistic nursing care on improving spontaneous delivery rate, promoting maternal and child health, and reducing complications. Methods From January 2006 to December 2008 720 cases of unipara ( in general nursing care) who were single cephalic without obvious indications of cesarean section were taken as control group. From January 2009 to December 2011 1 020 cases of unipara (in humanistic nursing care) who were also single cephalic without obvious indications of cesarean section were taken as observation group. The rates of vaginal delivery and cesarean section, and incidences of postpartum hemorrhage, fetal distress and neonatal asphyxia in two groups were compared. Results The spontaneous delivery rate of observation group was higher than that of control group (89.90% vs 64.17%, χ2 = 170.010, P 〈0.05), but the incidences of postpartum hemorrhage (1.71% vs 3.27% ,χ2 = 5. 096, P 〈 0.05), cesarean section ( 10.10% vs 35.83%, χ2 = 170. 010,P 〈 0.05), fetal distress (3.08% vs 4.92% ,χ2 = 19. 010, P 〈 0.05) and neonatal asphyxia ( 1.08% vs 3.06%, χ2= 8. 868, P 〈 0.05 ) were lower. Conclusion Humanistic nursing care is suitable for most puerperants, and it will play an important role in protecting and assisting spontaneous delivery, improving service quality, and ensuring health of mother and infant. It is worth to be spread.
