摘要 目的:观察不同程度疼痛对定量药物脑电图的影响。方法:15例硬膜外麻醉下行右胫骨骨折切开内固定术患者,于术后不同时间随访患者。采用视觉模拟评分法(visual analogue scale,VAS)对疼痛程度进行评估:0分:无痛;1~3分:轻度疼痛;4~6分:中度疼痛;7~10分:重度疼痛。并用定量药物脑电图(Quantitative pharmaco-EEG,QPEEG)采集脑电信号,直至患者无痛或出院。利用功率谱分析技术,计算QPEEG各频段的相对功率百分比。结果:疼痛可引起脑电的显著改变:大多数脑区的δ频段功率百分比增加;左额区θ频段功率百分比增加;两额及左顶、枕区的α频段功率百分比降低以及左顶、颞区β波活动增加(P<0.05)。结论:疼痛引起了脑电的显著改变,为QPEEG用于疼痛监测提供了可能性。 Objective To observe the effects of pain in different intensity on quantitative pharmaco-EEG (QPEEG). Methods 15 patients receiving tibial fracture surgery under epidural anesthesia were interviewied in different time of post operation. The intensity of pain was valued by visual analogue scale (VAS) : 0 : no pain ; 1 ~ 3: mild pain; 4 ~ 6: moderate pain; 7 - 10: severe pain. The EEG waves were recorded via QPEEG until patients without pain or discharge. The QPEEG percentages of relative power in each frequency band were calculated by technique of the power spectral analysis. Results The percentage of relative power in frequency bandincreased in most cerebral zone, 0 frequency band increased in lefl-fronto zone, ot frequency band decrease in fronto and left-biased cupula-occipital zone, wave activity increased in lefi-sided cupula-temporalzone. Conclusion Pain could cause EEG significantly change, which providing the possibility of monitoring pain by QPEEG.
