摘要
目的:探讨不同年龄段高血压患者肾功能水平与大动脉弹性之间的关系。方法:入选2008年10月至2010年10月安贞医院高血压科门诊及住院的高血压患者,通过MDRD公式估算肾小球滤过率(eGFR),评估肾功能,测定肱踝动脉脉搏波传导速度(baPWV)作为评估大动脉弹性功能的指标。对全部高血压病患者按照年龄分组,每10岁设为一组,共6组,比较不同年龄组之间eGFR与PWV的差异;并分别做eGFR和PWV与其他影响因素之间的简单相关分析。结果:437例患者(全部未服药),年龄20~77岁,eGFR与PWV在不同组之间比较和组间比较,差异均有统计学意义(F值分别为39.02和14.58,P<0.05)。eGFR水平与年龄、SBP、DBP、脉压、PWV及UA呈负相关(P<0.01);PWV水平与年龄、SBP、DBP及脉压呈正相关,与eGFR呈负相关,P<0.01。结论:不同年龄段高血压患者eGFR的降低和PWV增高是同等程度并行发生,二者之间关系密切。
Objective: To investigate the relationship between the level of renal function and large arterial elasticity in patients with hypertension of different ages.Methods: The patients with hypertension were enrolled from Anzhen Hospital hypertension outpatient and inpatient between October 2008 and October 2011.The glomerular filtration rate(eGFR) was estimated by the MDRD formula to assess the renal function,and the brachial ankle arterial pulse wave velocity(baPWV) was measured as the indicator for assessment of arterial elasticity.All hypertensive patients were grouped according to age,the age of each difference of ten years was set as a group and there were six groups in all.The differences between eGFR and PWV in the different age groups were compared,and the relationship between eGFR and PWV and other influencing factors were evaluated with univariate analysis.Results: Information on clinical data were obtained from 437 patients(all without medication),aged from 20 to 77 years.eGFR and PWV between different groups and between groups were statistically significant(F values were 39.02 and 14.58,P 0.05).eGFR level was negatively correlated with age,SBP,DBP,PWV,and UA(P〈0.01).PWV was positively correlated with age,SBP,DBP and pulse pressure,and was negatively correlated with eGFR.Conclusion: For patients with hypertension of different ages,the reduction of eGFR and the increase of PWV were occurred in the same degree in parallel,and both of them were closely related.
