摘要
目的探讨肺炎克雷伯菌感染的临床分布及耐药性分析,为临床合理用药提供依据。方法收集2010年5月-2011年8月医院门诊及住院患者送检的标本进行分离培养,菌株鉴定及药敏试验采用ATB Expression全自动细菌鉴定仪,产ESBLs检测采用双纸片增效法,对其分布及抗菌药使用进行分析。结果分离出182株肺炎克雷伯菌,产ESBLs检出53株,检出率为29.12%;痰液中检出率最高,占48.90%;主要分离自ICU,占30.77%,呼吸内科次之,占26.37%;产ESBLs菌株对多数抗菌药物的耐药率明显高于非产ESBLs菌株(P<0.01)。结论肺炎克雷伯菌,尤其是产ESBLs肺炎克雷伯菌临床耐药率高,应根据药敏结果结合临床感染情况合理使用抗菌药物,有效控制耐药菌的产生。
OBJECTIVE To analyze the clinical distribution and drug resistance of Klebsiella pneumoniae causing nosocomial infections so as to provide basis for the clinical reasonable use of antibiotics. METHODS The specimens submitted from the outpatient department and the hospitalized patients were isolated and cultured from May 2010 to Aug 2011, the bacterial identification and the drug susceptibility testing were performed by adopting ATB expression automatic microorganism analyzing system. Double disk synergy test was adopted in the strains with extended ESBLs; the distribution and the use of antibiotics were analyzed. RESULTS There were 182 strains of K. pneumoniae isolated among which there were 53 ESBLs-producing strains with the detection rate of 29.12%; the strains were mainly isolated from the sputum, accounting for 48.90%; the isolates mainly distributed in ICU (30. 77%), followed by the respiratory medicine department (26. 37%); the drug resistance rate of ESBLs- producing strains was significantly higher than that of non-ESBLs-producing strains (P〈0.01). CONCLUSION The drug resistance rate of K. pneumoniae, ESBLs-producing K. pneumoniae in particularly, is high; it is necessary to reasonably use antibiotics in accordance with the result of the drug susceptibility testing so as to effectively control the drug resistant strains.
