摘要
目的评价可降解泪小管栓塞治疗准分子激光原位角膜磨镶术(Lasik)术后患者干眼症的临床效果。方法于Lasik术后2～4周时,选取干眼症患者40例(80眼),随机分成2组,每组20例(40眼)。治疗组使用VisiPlug栓塞双侧下泪小管,并根据患者干眼症状酌情使用人工泪液;对照组仅使用人工泪液。2组分别于术前、术后3个月和6个月时检查干眼主观症状、泪膜破裂时间、基础泪液分泌量和角膜荧光素染色,观察泪小管栓塞对患者的干眼症状与体征的影响。结果所有泪小管栓塞术后的患者干眼症状和体征均明显减轻,与对照组比较差异有统计学意义(P<0.05)。大部分患者术后3个月时无需配合人工泪液治疗。2例患者术后3个月时出现轻度泪溢,未行特殊处理,术后6个月泪溢症状消失。观察期间未见泪小管炎及泪小管塞脱出。结论泪小管栓塞能有效地缓解和改善Lasik术后患者的干眼症状,并能以减少人工泪液用量,提高患者视觉质量。
Objective To evaluate the effects of degradable lacrimal plug on treating dry eye after Lasik.Methods 2-4 weeks after Lasik surgery,the subjective symptoms,Schirmer＇test Ⅰ,BUT test and fluorescein staining of the cornea were examined.Forty patients（80 eyes） diagnosed with dry eye were included in this study.The lower lacrimal ducts on both sides were occluded with degradable VisiPlug.The dosage of artificial tears was used based on the sensation of dry eye.Three months and six months after surgery,the symptoms and signs of dry eye were followed up.Results Dry eye sensation was significantly improved after plugging with lacrimal plug.The signs of dry eye were also significantly reduced.Most patients didn＇t need further artificial tears three months after surgery.Neither canaliculitis nor emersion of the lacrimal plug was found.Two patients had slight epiphora three months after plugging and resolved 6 month after surgery without any additional treatment.Conclusion Plugging of the lacrimal duct could reduce dry eye after LASIK surgery and improve vision quality.
