摘要
目的:观察非手术治疗对无神经损伤胸腰椎骨折的临床疗效。方法:选择我院收治的26例无神经损伤的胸腰椎骨折患者,采用体位复位和腰背支具的非手术治疗,伤后随诊1年,比较治疗前后脊柱后凸Cobb角、椎体前缘压缩比例、疼痛及工作状态的恢复情况。结果:所有患者在随访过程中神经系统检查均正常;治疗前患者Cobb角和椎体前缘高度的压缩比例分别为17.2°±2.2°和32.3%±4.7%,治疗后1周Cobb角明显减小,椎体前缘高度的压缩比例明显增加,但治疗后1年这两项指数与治疗前差异无统计学意义(P>0.05);治疗前患者均伴有中-重度疼痛,部分或完全丧失工作能力,治疗后大部分患者疼痛缓解和恢复工作状态。结论:对于无神经损伤的胸腰椎骨折,采用体位复位和腰背支具的非手术治疗虽然不能长期矫正脊柱的后凸畸形,但对缓解症状和恢复工作状态疗效较好,总的来说是治疗胸腰段脊柱骨折较好的方法。
Objective：To investigate the clinical effect of the treatment of vertebra fractures by nonoperative treatment. Methods： Twenty-six consecutive patients with thoracolumbar vertebra fracture were enrolled in our study. All of the patients were managed with nonoperative treatment （ application of a body cast or thosis）. Bisegmental Cobb angle, anterior vertebral body height ratio,level of pain and physical ability were analyzed before and after treatment. Results：All patients had normal findings on a neurological examination performed at the time between pre-treatment and final follow-up. The bisegmental Cobb angle and anterior vertebral body height ratio were 17.2°±2.2°and 32.3% ±4.7% in all of the patients respectively. The Cobb angle was reduced and the anterior vertebral body height ratio were improved after treatment for one week, but this improvement was retrogressed to the condition at admission. All pa- tients had moderate-to-severe pain and partial-to-complete physical disability before treatment. Whereas the pain was relieved as well as the physical ability were restored to the normal in most of the patients after treatment for one year. Conclusion：Although nonoperative treatment （application of a body cast or tbosis） have no advantage on correlation of the malformation,whereas it can relieve the symp- tom and restore the physical ability in patients with thoracolumbar vertebra fracture. In general,nonoperative treatment （application of a body cast or thosis） is a preferable method for thoracolumbar vertebra fractures.
