摘要
目的探讨重组人脑利钠肽(rhBNP)治疗急性心肌梗死后心力衰竭的临床疗效。方法将急性心肌梗死伴心力衰竭患者98例纳入研究,按入院的先后顺序随机分成观察组与对照组,每组49例。均给予吸氧、低盐饮食、强心、利尿、抗感染、治疗原发病等常规治疗措施。在常规治疗基础上,对照组给予静脉滴注硝酸甘油而观察组给予静脉滴注rhBNP,比较两组用药后疗效。结果治疗1天后,心功能改善的总有效率,观察组为87.3%,而对照组为61%;整体临床状况好转程度,观察组比率为72.7%,而对照组为45.5%;观察组的呼吸困难程度评分为(2.68±0.43)分,EF值为(42.25±16.94)%,Tei指数为(0.56±0.03),尿量(1835.7±349.5)ml;而对照组的呼吸困难程度评分为(3.85±0.72)分,EF值为(40.73±15.17)%,Tei指数为(0.50±0.02),尿量(1638.5±333.8)ml,组间比较差异有统计学意义(P<0.05)。结论 rhBNP能有效改善急性心肌梗死后心力衰竭患者的呼吸困难和临床状况,提高心功能改善的总有效率,安全可靠,优于硝酸甘油,值得在临床推广应用。
Objective To observe the effect of recombinant human brain natriuretic peptide (rhBNP) on the treatment of acute myocardial infarction with heart failure. Methods By order of admission, 98 patients of acute myocardial infarction with heart failure were randomly divided into observation group and control group (n= 49). All were given oxygen, low salt diet, cardiac, diuretic, anti-infection, the treatment of primary disease and other conventional treatment. On the basis of the conventional treatment, the control group received intravenous infusion of nitroglycerin intravenous nitroglycerin while the observation group with intravenous infusion rhBNP. After treatment, efficacy were compared between the two groups. Results After treatment, the total efficient of the improvement of cardiac function was 87.3% for the observation group and 61% for the control group; the overall clinical status improve ratio was 72.7 % for the observation group and 45.5% for the control group; the degree for the difficulty in breathing, EF value, Tei index and urine were (2.68±0.43) points, (42.25±16.94)%, (0.56±0. 03), and (1835.7±349.5)ml respectively for the observation group, while in the control group, the degree for difficulty in breathing, EF value, Tel index and urine were (3.85± 0.72) points, (40. 73±15.17)%, (0. 50±0. 02) and (1638.5±333.8)ml respectively, the inter group difference was statistically significant (P〈0.05). Conclusion RhBNP can effectively improve the dyspnea and the clinical status of patients with heart failure after acute myocardial infarction, it could enhance the improvement of cardiac function efficiently, it's safe and reliable, superior to nitroglycerin, and it is worthy of clinical application.
