功能磁共振成像联合弥散张量纤维束成像在脑肿瘤手术中的应用价值

The application of fMRI combined with DTI in cerebral tumors resection

摘要 目的探讨功能磁共振成像(fMRI)和弥散张量纤维束成像(DTI)技术对提高脑肿瘤的全切率的作用。方法 16例脑肿瘤患者为观察组,手术前后行行fMRI和DTI检查;21例脑肿瘤患者为对照组,未行fMRI与DTI检查。两组均在神经导航下显微手术切除肿瘤,比较肿瘤全切率及致残率,观察两组手术前后Karnofsky功能评分(KPS评分)。结果观察组肿瘤全切除率明显高于对照组(<0.05),两组术后神经功能改善率差异无统计意义(>0.05)。结论 fMRI及DTI可以立体形象地显示肿瘤对脑功能区及白质纤维束的影响,有助于提高脑肿瘤在导航下手术全切率,并有可能降低其术后致残率。 Objective To implore the value of fMRI combined with DTI to improve the rate of the total tumor resection.Methods 16 cases of brain tumors were treated as observed group with fMRI and DTI examination Pre-and postoperation;21 patients without fMRI and DTI examination were treated as control groups.The tumors in the two groups were resected by the neuronavigator-assisted microsurgery, the rates of total resection of the tumors and disability after the operation were compared between the groups. The preoperative and postoperative KPS were respectively acquired to evaluate the condition of patients. Results The rate of total tumor resection in observed group were obviously higher than the control group(P〈0.05) ,the difference of the nerve function improvement after operation in the two groups showed no statistical significance (P〉0.05). Conclusion The use of DTl combined with fMRI can display the effects of tumor on brain functional and white matter fiber tracts.The rate of total resection of tumors can be increased by the neuronavigator-assisted microsurgery, and the rate of morbidity after operation can probably be reduced.
