飞行人员腰椎终板骨软骨炎的影像学分析

摘要 目的:探讨飞行人员腰椎终板骨软骨炎影像学表现及飞行时间和飞行机种的关系。方法:回顾性分析31例飞行人员腰椎终板骨软骨炎影像学表现:飞行时间分成〈1000h、1000~3000h、〉3000h等3组。结果:31例患者中DR提示腰椎体上缘或下缘骨质硬化8例(25.8%);MRI共累计69个终板,多发者14例(45.2%),单发者17例(54.8%),主要发生部位在L4/L5、L5/S1相对缘。飞行时间在1000~3000h组的飞行人员腰椎终板骨软骨炎患病率高于〈1000h组及〉3000h组。结论:飞行人员腰椎终板骨软骨炎主要发生在L4/5、L5/S1相对缘,是引起飞行人员腰痛的原因之一。飞行人员在飞行黄金阶段应加强自我保护。 Objective To investigate radiographic findings of pilots with lumbar end-plate osteochondritis and the relationship of flight time and machine. Methods Radiographic findings of 31 cases of pilots with lumbar end-plate osteochondritis were retrospectively analysed, and three groups were generated, which had the flight time less than 1 000 h, 1 000-3 000 h and more than 3 000 h respectively. Results Eight cases(25.8%) were diagnosed with bone hyperplasia of vertebral margin by DR and 69 end-plates were involved by MRI in 31 cases of lumbar end-plate osteochondritis, there were 14 cases(45.2%) suffered from multiple end-plate osteochondritis and 17 cases(54.8%) from single end-plate osteochondritis, most of whom were located at adjacent vertebral margin of IA/5 and LS/S1. Mobidity of lumbar end-plate osteochondritis of the pilots with the flight time between 1 000 and 3 000 h was higher than that of less than 1 000 h and more than that of 3 000 h. Conclusion Most of pilots with lumbar end-plate osteochondritis are located at adjacent vertebral margin of L4/5 and LS/S1, which is one of the main reasons causing low back pain.
