Avastin联合周边视网膜冷冻术治疗晚期新生血管性青光眼 被引量:4

摘要 目的回顾性研究Avastin玻璃体内注射联合周边视网膜冷冻术治疗晚期新生血管性青光眼的效果。方法随机选择2006年5月至2010年12月在我院住院的73例(73眼)晚期新生血管性青光眼患者,分为A、B两组:A组(45例45眼)患者玻璃体内注射Avastin 0.05mL(1.25mg)联合周边视网膜冷冻术;B组(28例28眼)患者仅行周边视网膜冷冻术。术后随访6个月,对比观察患者手术前后虹膜新生血管、视力、眼压变化及眼底、角膜内皮细胞计数情况,并对两组各时间点患者的眼压及角膜内皮细胞计数进行比较。结果 A组患者术后第1天虹膜新生血管明显减少、变细,术后第3天17例(17眼)患者虹膜新生血管消失,房角新生血管萎缩,其余患者虹膜及房角表面新生血管明显萎缩变细,隐约可见,术后2周虹膜及房角新生血管消失;B组患者术后第3天及2周时虹膜新生血管明显可见,术后1个月新生血管消失。A组患者术后各时间点眼压控制好,术后2周、1个月、3个月、6个月分别为18.24mmHg(1kPa=7.5mmHg)、17.48mmHg、17.35mmHg和16.87mmHg,明显好于B组相应时间点眼压(分别为27.07mmHg、21.23mmHg、19.36mmHg18.39mmHg);A组患者术后2周、1个月、3个月、6个月角膜内皮细胞计数分别为1527.36mm-2、1548.25mm-2、1502.34mm-2和1532.45mm-2,明显多于B组(分别为1432.54mm-2、1354.67mm-2、1389.24mm-2、1364.57mm-2)。结论玻璃体内注射Avastin可明显减轻晚期新生血管性青光眼患者症状,加快新生血管消退,降低患者眼压,保护角膜内皮细胞;联合周边视网膜冷冻术可作为晚期新生血管性青光眼的有效治疗方法。 Objective To study the effects of Avastin combined with peripheral retinal coagulation for advanced neovascular glaucoma retrospectively. Methods A total of 73 neovascular glaucoma patients(73 eyes) were selected in our hospital from May 2005 to December 2010,and divided randomly into group A and B. In group A(45 cases,45 eyes) ,0.05 mL Avastin( 1.25 mg) were injected and peripheral retinal coagu- lation were performed. In group B ( 28 cases, 28 eyes), only peripheral retinal coagula- tion were carried out. The follow-up time was 6 months, and the preoperative and post- operative iris neovascularization, visual acuity, intraocular pressure and corneal endo- thelial cell counting were observed and compared. Ill,suits The iris neovasculariza- tion at postoperative 1 day in group A obviously reduced and thinned, which disap- peared in 17 patients at postoperative 3 days, the angle neovascularization was atro- phied, and disappeared in all patients 2 weeks later, however these in group B were ob- viously seen at postoperative 3 days and 2 weeks, and disappeared 1 month later. The in- traocular pressure in group A was controlled better at each postoperative time point, which at postoperative 2 weeks,1 month,3 months and 6 months were 18.24 mmHg( 1 kPa = 7.5 mmHg), 17.48 mmHg, 17.35 mmHg and 16.87 mmHg, respectively, obviously lower than those in group B(27.07 mmHg,21.23mmHg, 19.36 mmHg and 18.39 mmHg). The corneal endothelial cell cotmting in group A at postoperative 2 weeks, 1 month,3 months and 6 months were 1527.36 mm-2 ,1548. 25 mm-2 ,1502. 34 mm-2 and 1532.45 mm-2 ,respectively,obviously more than those in group B( 1432.54 mm-2, 1354.67 mm-2 ,1389.21 mm-2 ariel 1364. 57 mm-2). Conelusion Intraocular injection Avastin can relieve the pain of advanced neovascular glaucoma, accelerate the iris neovascularization disappear, decrease intraocular pressure and preserve the corneal endothelial cell.
