摘要
目的:探讨锁骨钩钢板治疗锁骨远端骨折临床效果及安全性。方法:收治缩骨远端骨折患者90例,采用随机数字表法分为克氏针张力带组和锁骨钩钢板组,每组45例,分别采用肩峰、锁骨远端双侧克氏针张力带内固定及锁骨沟钢板内固定治疗;比较两组内固定优良率、肩关节功能改善优良率,预后优良率,手术时间,术中出血量、住院时间、骨折愈合时间以及并发症发生情况等。结果:锁骨钩钢板组内固定优良率明显高于克氏针张力带组,组间比较差异显著(P<0.05);锁骨钩钢板组术后肩关节功能改善优良率明显高于克氏针张力带组(P<0.05);锁骨钩钢板组预后优良率均明显高于克氏针张力带组(P<0.05);锁骨钩钢板组住院时间及骨折愈合时间均明显短于克氏针张力带组(P<0.05);两组手术时间和术中出血量组间比较无显著差异(P>0.05);锁骨钩钢板组住院时间及骨折愈合时间均明显短于克氏针张力带组(P<0.05);同时锁骨钩钢板组并发症发生率明显低于克氏针张力带组(P<0.05)。结论:锁骨钩钢板治疗锁骨远端骨折能够有效促进肩关节功能恢复,改善预后,缩短病程,减轻痛苦,且并发症发生风险更小。
Objective：To investigate therapeutic effect and security of clavicular hook plate on distal clavicular fracture. Methods： 90 patients with distal clavicular fracture. were chose in the period from March 2009 to October 2011 in our hospital and randomly divided into two groups including Kirschner wire fixation group （45 patients） and clavicular hook plate group （ 45 patients ） ; and the excellent and good rate of fixation, shoulder function and prognosis, the operation time, the blood loss, the time of staying hospital, healing time of fracture and the complication of both groups were compared. Results：The excellent and good rate of fixation, shoulder function and prognosis, the time of staying hospital and healing time of fracture of clavicular hook plate group were significantly better than Kirschner wire fixation group （P 〈 0. 05 ）. There was no significant difference in operation time and blood loss （P 〉 0. 05） ;And the complication incidence of clavicular hook plate group were significantly better than Kirsehner wire fixation group （ P 〈 0. 05 ）. Conclusion ： The clavicular hook plate on distal clavicular fracture can effectively improve the shoulder function and prognosis, shorten the illness duration, relieve the pain, and have less complication risk.
