摘要
目的分析影响阿尔茨海默病(AD)的相关危险因素。方法在宁波市北仑区中医院和宁波市康宁医院2009年1月～2010年12月间就诊的阿尔茨海默病患者,共入组78例,对照组按照性别、年龄(±3岁)、文化程度作为配对条件,1∶2配比在社区选择健康老年人入组,完成基线调查。包括简易精神状态量表(MMSE)、总体衰退量表、Hachinski缺血指数量表、临床痴呆评定量表等评定。结果多因素分析证实:参与社会活动,阅读或写作,棋牌或麻将,躯体疾病,健身、悠闲生活,丧偶/离异,家族史与阿尔茨海默病的发生相关,OR值(95%CI)分别为0.413(0.264～0.783),3.572(2.674～5.828),0.512(0.316～0.869),2.265(1.543～3.474),0.761(0.451～0.946),2.738(1.414～4.609)、1.532(1.016～2.316)。结论参与社会活动、阅读或写作、棋牌或麻将、健身悠闲生活是阿尔茨海默病的保护因素,而躯体疾病、丧偶/离异、家族史是影响阿尔茨海默病的重要危险因素。
Objective To assess possible risk factors associated with Alzheimer disease （AD） among senior citi zens in Ningbo. Methods A total of 78 clinically diagnosed AD patients and 156 healthy controls matched by age, sex and education from Ningbo urban communities were randomly enrolled into the study. All subjects were required to complete a self-compiled questionnaire and were assessed by simple mental state examination （MMSE）, Global Deterioration Scale, Hachinski Isehemic Scale and Clinical Dementia Rating. Chi-square test and conditional logistic regression were used to analyze the data. Results Multivariate analysis revealed that factors significantly associated with AD were social activities, reading/writing, playing chess, physical illness, physical, exercises, being divorced/widowed and family history with the OR value （ 95% confidence interval） of 0.413 （0.264-0.783）, 3.572 （2.674-5.828）, 0.512 （0.316-0.869）, 2.265 （1.543-3.474）, 0.761 （0.451-0.946）, 2.738 （1.414-4.609） and1.532 （1.016-2.316）, respectively. Conclusions Socialactivi ties, reading/writing, playing chess and physical exercises are found to be positive factors associated with AD, while physical illness, being widowed and family history are negative ones.
