摘要
目的通过对1,582例临床确诊的白血病患者外周全血标本的检测,研究Sysmex XE-2100散点图的分布趋势,建立一套用于白血病FAB分型系统初筛的方法,从而减少临床初诊时对相关血液系统疾病的误诊与漏诊。方法利用Sysmex XE-2100全血细胞分析仪检测外周全血标本,并对XE-2100系统中DIFF通道、BASO通道以及IMI通道三部分的散点图进行综合评估。结果DIFF通道与IMI通道可以将粒系统白血病与淋巴系统和单核系统白血病进行区分,识别率分别为94.8%、96.8%、81.3%;对L1型、L2型、L3型的识别率分别为30.6%、34.8%、33.5%;对M1型、M2型、M3型、M4型、M5型、M6型、M7型的识别率为62.9%、44.5%、44.8%、47.0%、40.3%、30.9%、0%。结论XE-2100散点图可以用于粒系统、单核系统及淋巴系统白血病的初步鉴别,可以对M1、M2、M3进行初步鉴别,但无法对单核系统M4与M5、及淋巴系统白血病进行鉴定,其他系统及类型的白血病无法应用。
Objective To determine the whole blood specimen in 1,582 cases leukemia patients with confirmed diagnosis,study the distribution and tendency of scatter graph of Sysmex XE-2100 analyzer, and establish a primary screening method for leukemia FAB typing,thereby to reduce the misdiagnosis of the blood system diseases when first visited.Methods We determined the specimens with Sysmex XE-2100 analyzer, evaluated the scatter graph of DIFF, BASO,and IMI channel in XE-2100 system.Results We can differentiate the granulocyte leukemia from monocytic leukemia and the lymphocytic leukemia in the DIFF and IMI channels,the identification ratio are 94.8%,96.8% and 81.3%;the identification ratio to L1,L2 and L3 are 30.6%,34.8%,33.5%;the identification ratio to M1,M2,M3,M4,M5,M6 and M7 are 62.9%,44.5%,44.8%,47.0%, 40.3%,30.9%,0%.Conclusion We can differentiate the granulocyte leukemia from monocytic leukemia and the lymphocytic leukemia,and can differentiate M1,M2 and M3,but can not differentiate all types of lymphocytic leukemia,and can not differentiate M4 from M5.
