摘要
患者男,30岁。面、项部红斑、丘疹伴瘙痒15d,泛发全身13d。体检:T37.4℃,全身浅表淋巴结肿大,咽充血,咽腭弓瘀点,双侧扁桃体I°肿大。皮肤科情况:头部散在分布红斑;项部密集分布丘疹、斑丘疹,融合成斑块;颈部、躯干、四肢丘疹及暗红斑,双小腿伸侧散在紫癜;掌、跖部暗红斑,肿胀明显,轻触痛。外周血异形淋巴细胞8%,血生化示肝酶升高,EB病毒抗体阳性,彩色多普勒检查示脾大。诊断:成人传染性单核细胞增多症。经地塞米松等治疗,患者痊愈。
A 30-year old man developed erythema and papules with itching on the face and the nape 15 days ago, and generalized 13 days ago. PE： T 37.4 ℃. Physical examination showed generalised lymphadenopathy, hy- peremia and petechia on the arcus palatinus posterior pharynx. Erythema, papules, maculopapules and sact- tered plaques were all over the body, and some purpura were on shins. The result of blood routine showed the rate of atypical lymphocytes was 8%, blood biochemistry showed increased hepatic enzymes, and EBV- VCA-IgM was positive. Color Doppler examination revealed splenomegaly. A diagnosis of adult infectious mononucleosis was made. The patient was cured mainly by dexamethasone.
