后路钉棒系统内固定结合后外侧植骨治疗胸腰段脊椎骨折 被引量:7

Posterior Surgery Pedicle Screw/Rod Instrumentation System Combined with Posterolateral Bone Treating Thoracolumbar Vertebral Fractures

摘要 目的观察后路钉棒系统内固定结合后外侧植骨治疗胸腰段脊椎骨折的临床应用效果。方法选取我院因胸腰段脊椎骨折住院患者63例采用后路钉棒系统内固定结合后外侧植骨治疗胸腰段脊椎骨折,随访8~18个月,比较分析伤椎前后缘高度、Cobb角及神经功能恢复情况。结果术后伤椎前后缘高度[分别为(24.1±2.3)mm、(25.6±1.5)mm]较术前[分别为(9.7±2.1)mm、(18.9±1.9)mm]明显增大,Cobb角(2.1±1.1)°较术前[(17.9±2.8)°显著减小,经统计学分析,差异具有显著性(P〈0.05),具有可比性;除7例全瘫患者,其余患者脊髓神经损伤均有不同程度的功能恢复。结论后路钉棒系统内固定结合后外侧植骨手术治疗能有效的使伤椎复位,重建椎体的稳定性,维持脊柱的正常生理功能,疗效肯定,并且操作简单,对患者损伤较小,值得临床推广使用。 Objective To observe the clinical application effect of posterior surgery pedicle screw/rod instrumentation system combined with posterolateral bone treating thoracolumbar vertebral fractures.Method 63 patients of thoracolumbar vertebral fractures hospitalized in our hospital were selected and treated by posterior surgery pedicle screw/rod instrumentation system combined with posterolateral bone All the patients were followed up for 8~18 months.The front and rear edge height of the injured vertebra,cobb angle and recovery of neurological function were comparativly analyzed.Results After operation,the result of front and rear edge height of the injured vertebra significantly increased(24.1±2.3mm,25.6±1.5mm vs 9.7±2.1mm,18.9±1.9mm),cobb angle significantly reduced(2.1±1.0° vs 17.9±2.8°).The dif ferences were significant(P0.05).Except for the seven cases of paralysis patients,the remaining patients with spinal cord injuries had different degrees of functional recovery.Conclusion Posterior surgery pedicle screw/rod instrumentation system combined with posterolateral bone can reset the fractured vertebral effectively,reconstruction of the stability of the vertebral to maintain the normal physiological function of the spine.The operation is easy to operate and has a adefinite effect small injury.It is worth spreading.
