摘要
目的 观察体外循环 (CPB)心脏瓣膜置换术前后病人血中多种细胞因子水平及体外诱生能力的变化。方法 30例择期行心脏瓣膜置换术的病人 ,于术前及术后第 1、3、7、14d检测血中白细胞介素 1(IL 1)、白细胞介素 2 (IL 2 )、白细胞介素 4(IL 4)、白细胞介素 8(IL 8)、肿瘤坏死因子α(TNFα)、和可溶性白细胞介素 2受体 (SIL 2R)水平及其体外诱生能力。结果 IL 1和SIL 2R在CPB后第 1、3d显著性增高 (P <0 .0 1或 0 .0 5 ) ,第 7d则明显降低 (P <0 .0 5 ) ;IL 2在CPB后第 1~ 7d明显降低 (P <0 .0 1或 0 .0 5 ) ;IL 8在CPB后第 1~ 14d显著增高 (P <0 .0 1) ;TNFα在CPB后无显著性变化 ;各时点的IL 4均未检测出。体外诱生水平变化 :IL 2、IL 4和SIL 2R在CPB后第 1~ 7d显著减少 (P <0 .0 1或 0 .0 5 ) ;IL 1在CPB后第 1d明显增加 (P <0 .0 1) ,第 3~ 7d则显著减少 (P <0 .0 1) ;IL 8在CPB后第 1、3d明显增加 (P <0 .0 1) ;TNFα仅在CPB后第 7d显著减少 (P <0 .0 5 )。
Objective To determine the changes of blood cytokines values and inductions of cytokines expressions after CPB and cardiac valve replacement. Methods Thirty patients undergoing cardiac valve replacement were randomly selected, The plasma cytokines levels or activities and inductions of cytokine expressions were measured before CPB, 1st, 3rd, 7th, 14th d after operation.Results Compared with the baselines, IL 1 activity and souluble interleukin 2 receptor(SIL 2R) level increased on the 1st, 3rd d , then decreased significantly on the 7th d, plasma IL 8 level increased in 14 d after CPB, but IL 2 activity decreased during 7 d postoperatively(P<0.01 or 0.05), and plasma TNFα did not change markedly (P>0.05); induction of IL 1 expression increased significantly on the 1st d and decreased on the 3rd d (P<0.01),induction of IL 8 expression was significantly elevated on the 1st and 3rd d (P<0.01), inductions of IL 2, IL 4 and SIL 2R expressions decreased significantly ofter CPB(P<0.01 or 0.05), and induction of TNFα expression only decreased on the 7th d. IL 4 was not detectable during the whole procedures.Conclusions The blood cytokines values and inductions of cytokines expressions change greatly in patients after cardiac valve replacement with CPB.
