摘要
目的观察改良Parks切口联合外直肌后徙调整缝线术治疗共同性外斜视的临床效果。方法采用改良Parks结膜切口联合外直肌后徙调整缝线术治疗共同性外斜视46例,术后随访3～24个月,观察治疗效果。结果 46例患者中13例术后未达到预期效果,欠矫4例,过矫9例,手术后24～48 h内于表面麻醉下进行眼位调整,13例均将眼位调整至预期效果。随访3～24个月显示,未调整者欠矫2例,过矫2例,调整者欠矫2例,余40例正位。所有患者术后球结膜切口对合整齐,轻度充血、水肿,术后5～8 d完全愈合,睑裂部无明显瘢痕。4例患者调整缝线结对应的结膜面局部结节状隆起水肿,给予0.1%氟美瞳眼水点眼后消退。未见感染、巩膜穿通、肌肉滑脱等并发症。结论采用改良Parks结膜切口联合外直肌后徙调整缝线术在提高手术的准确性和减少手术并发症上有诸多的优越性。
Objective To observe the clinical effect of lateral rectus recession and adjustable suture surgery combined with modified parks conjunctival incision in the treatment of concomitant exotropia. Methods Using modified parks conjunetival inci- sion combined with lateral rectus recession and adjustable suture surgery in the treatment of concomitant extropia in 46 cases, the follow-up time were 3 - 24 months after operation, and the curative effects were observed. Results In the 46 patients, 13 cases were not initially in desired alignment, under correction in 4 cases, overcorrection in 9 cases. In the 24 - 48 hours after surgery, 13 cases had adjustment, they were to be adjusted to the expected effect of eye position. The results after follow-up 3 -24 months ： There were 2 cases of extropic and 2 eases of esotropic in patients without adjusted and there were 2 cases of extropic and 40 cases of orthotropic in patients with adjusted. Healing of conjunctival incision was good. All conjunctival wounds were healed 5 -8 days after surgery. There was no obvious eonjunctival scarying in lid fissure. Four patients had local swelling chemosis a- bove suture tie and cured with topical eorticosteroid. No complication such as infection, seleral penetrating or muscle slippage. Conclusion There are many superiority for modified Parks conjunctival incision combined with adjustable suture in improve the operation accuracy and reduce the operation complication.
