骨水泥致兔急性肺血栓栓塞症早期血循环中致炎因子变化的实验研究

The change of proinflammatory cytokine levels in the circulaton following acute pulmonary thromboembolism induced by bone cement in rabbits

摘要 目的研究骨水泥(聚甲基丙烯酸甲酯,PMMA)栓子致兔急性肺栓塞后细胞因子(TNF-α、IL-6)水平变化及其意义。方法取成年健康日本大耳白兔24只,随机分为骨水泥栓塞组、自体血凝块栓塞组、对照组,每组8只,栓塞组经兔股静脉分别注入骨水泥栓子、自体血凝块栓子,制备肺血栓栓塞症(PTE)模型,并动态观察TNF-α、IL-6的变化。结果骨水泥栓子注入后TNF-α在0.5~6.0h呈逐渐升高趋势,与同期对照组、血凝块栓塞组比较差异均有统计学意义(P<0.05)。IL-6在0.5h与对照组、血凝块栓塞组比较差异无统计学意义(P>0.05),1~6h与同期对照组、血凝块栓塞组比较差异均有统计学意义(P<0.05)。血凝块栓塞组与同期对照组在各时点的TNF-α、IL-6比较,差异均无统计学意义(P>0.05)。结论单枚血凝块栓子致兔PTE后,在早期(6h内)未见TNF-α、IL-6的高表达,而骨水泥栓塞组引起兔血清TNF-α、IL-6升高。 Objective To investigate the change and significance of cytokine levels in acute pulmonary thromboembolism induced by bone cement in rabbits.Methods Twenty-four rabbits were randomly devided into 3 groups,8 rabbits were established pulmonary embolism model by injection of bone cement embolus into femoral vein,8 rabbits were injected autol ogous blood clot into femoral vein,another 8 rabbits as control group.Then the dynamical changes of tumor necrosis fac tor-α(TNF-α) and interleukin-6(IL-6) in blood were observed.Results TNF-α in blood were increased significantly after injection of bone cement embolus 0.5 to 6 hours,there was statistically significant difference between bone cement em bolus group and the both blood clot group and control group(P 0.05).There was no statistically significant difference about IL-6 in 0.5 h with the control group,blood clot embolization group(P 0.05);for 1-6 h with the same period in the control group,the blood clot embolization group,difference was statistically significant(P 0.05).There was no statistically significant difference about TNF-α,IL-6 in blood clot embolization group with the same period in the control group at each time point(P 0.05).Conclusion TNF-α,IL-6 in blood circulation is not significantly elevated in 6 h after PTE induced by one blood clot embplus,so it is bone cement to induce the changes of TNF-α,IL-6 in blood circulation.
