摘要
目的了解医患关系不协调事件对护士心理、工作意愿及接受相关处理方法与技巧培训意愿的影响。方法采用问卷调查方法,于2005年7月～2011年6月,选择在本院急诊、内科、外科及妇产儿科工作,亲身或未亲身经历医患关系不协调事件的54名护士。了解不同科室护士经历医患关系不协调事件的频率,医患关系不协调事件对不同科室护士心理及继续从事护理工作意愿的影响,不同年龄、工作年限、职称护士对接受相关处理方法与技巧培训的意愿。结果急诊科护士经历医患关系不协调事件频率最高,占64.8%(35/54),其次分别为妇产儿科37.0%(20/54)、内科25.9%(14/54)、外科13.0%(7/54)。发生1周内,急诊科护士心理反应最大,占81.5%(44/54),其次分别为妇产儿科70.4%(38/54)、外科50.0%(27/54)、内科38.9%(21/54)。对心理影响时间以内科护士最长,占59.3%(32/54),其次分别为外科50.0%(27/54)、妇产儿科29.6%(16/54)、急诊科20.4%(11/54)。在经历过医患关系不协调事件的护士中,曾经有不干临床护理工作想法的护士中,内科、外科、妇产儿科、急诊科室护士分别占81.5%(44/54)、70.4%(38/54)、29.6%(16/54)及9.3%(5/54)。对接受医患关系不协调事件相关处理方法与技巧培训中,年龄小、工作年限低、职称低的护士表现学习意愿更强烈(均P<0.05)。结论医患关系不协调事件影响着护士心理健康及从事临床护理工作的意愿,直接影响着护理队伍的稳定性,应加强相关知识的培训,提高护士处理医患关系的能力及技巧。
Objective To investigate the influence of discordant nurse-patient relationship on the mental conditions and the willingness to work and receive relevant treatment and skills training.Methods During July 2005 to June 2011,54 nurses from departments of emergency,internal medicine,surgery,gynecology and obstetrics,and pediatrics were involved in the investigation on the frequency of events related to discordant nurse-patient relationship,the influence of the events on the nurses＇mental conditions and their willingness to continue their job and their willingness to receive relevant treatment and skills training.Results 64.8%of the nurses（35/54）from emergency department nurses,37.0%（20/54）from the department of maternity and pediatrics,25.9%（14/54）from the department of internal medicine and 13.0%（7/54）from the department of surgery experienced the events.Within 1 week after the events,81.5%（44/54）of the nurses from emergency department,70.4%（38/54）from the department of maternity pediatric,50.0%（27/54）from the department of surgery,and 38.9%（21/54）from the department of internal medicine experienced psychological reaction.59.4%（32/54）of the nurses from the department of internal medicine,50%（27/54）from the department of surgery,29.6%（16/54）from the department of maternity and pediatrics and 19.7%（11/54）from the emergency department suffered from the psychological impact.Among those experiencing the events,81.5%（44/54）of them from the department of internal medicine,70.7%（38/54）from the department of surgery,30.0%（16/54）from the department of maternity and pediatrics and 9.1%（5/54）from the emergency department used to wish to resign their job.The nurses who were younger and had little work experience and lower job titles tended to have a stronger desire for training,compared to those older and experienced nurses（P0.05）.Conclusions The events related to discordant nurse-patient relationship may affect the mental health of nurses as wells as their wi
