摘要
目的了解维持性血液透析老年患者的营养状况,分析其影响因素。方法选取接受血液透析超过6个月的125例终末期肾病老年患者,收集患者的一般情况、临床特点及伴发疾病、膳食资料,测定患者的C反应蛋白、血红蛋白、血胆固醇指标,测评患者的尿素清除指数。采用主观综合营养评估法评定营养状况,分析营养不良的相关影响因素。结果维持性血液透析老年患者营养不良的患病率为48.8%。多因素Logistic回归分析确定蛋白摄入不足(P=0.013)、透析不充分(P=0.011)以及糖尿病(P=0.046)是维持性血液透析老年患者发生营养不良的独立危险因素。结论营养不良在维持性血液透析老年患者中存在较高的患病率。透析不充分、蛋白摄入不足以及伴发糖尿病可以增加维持性血液透析老年患者营养不良发生的风险。
Objective To understand the nutritional status of elderly maintenance hemodialysis (MHD) patients, and to study its influencing factors. Methods One hundred and twenty five elderly MHD patients were tested with Subjective Global Assessment (SGA) to assess their nutritional status, and their related factors were collected including clinical characteristics, biochemical parameters, dietary data. The influencing factors of malnutrition were analyzed. Results It was found that 48.8% of elderly MHD patients were malnutritional. The multivariate logistic regression analysis indicated that low protein intake (P=0.013), inadequate hemodialysis (P=0.011), and diabetes mellitus (P=0.046) were risk factors for malnutrition of elderly MHD patients. Conclusion There is a high prevalence rate of malnutrition in elderly MHD patients. Low protein intake, inadequate hemodialysis and diabetes mellitus may enhance malnutrition of elderly MHD patients.
