摘要
目的:探讨全膝关节置换术后疼痛原因。方法:对2010年11月至2011年6月共46例固定平台的全膝关节置换术后患者进行回顾分析。随访1 a。男性患者13例,女性患者33例。进行术后膝关节CT扫描。结果:46个患者中包括14个术后疼痛患者和32个无痛患者。疼痛组中胫骨假体平均内旋9.43°(外旋3°-内旋16°),股骨假体平均内旋6°(内旋2°-内旋10°);无痛组中是胫骨假体平均外旋0.44°(外旋8°-内旋6°),股骨假体平均外旋0.75°(外旋5°-内旋4°)。结论:全膝关节置换中假体可以适度外旋,而假体的内旋会导致全膝关节置换术后疼痛,尤其是在胫骨平台内旋大于6°,股骨假体内旋大于4°的情况下。
Objective: To investigate the postoperative pain of total knee replacement with fixed bearing.Methods: To performed 46 fixed bearing total knee arthroplasty from November 2010 to June 2011.A retrospective analysis was made on these 13 men and 33 women.They attend CT scans to determine the rotational alignment of the tibial and femoral components.Results: 14 painful prsented symptom and 32 have no pain in total 46 patients.In the painful group,the mean rotation of the tibial component was 8.36° of internal rotation(16° internal to 5° internal rotation),as to femoral rotation it was 6°of internal rotation(2° internal to 10° internal rotation;In the painfree group,the mean rotation of the tibial component was 0.44° of external rotation(8° external to 6° internal rotation),as to femoral component rotation it was 0.75°of external rotation(5° external to 4° internal rotation).Conclusion: External rotation of components in TKA is allowed.But internal rotation of the prosthesis will result in postoperative pain in total knee arthroplasty.Especially when tibial component rotation is bigger than 6° internal rotation or femoral component rotation is bigger than 4°internal rotation.
