乳腺大汗腺癌的临床病理特征及预后分析

摘要 目的探讨乳腺大汗腺癌的临床病理特征及预后。方法回顾性整理复习4例乳腺大汗腺癌患者的临床资料,对其进行组织学观察、特殊染色及免疫组化染色,并结合文献复习。结果本组4例均为女性,发病年龄45~58岁,平均51.5岁。临床上表现为乳腺无痛性肿块,质地坚硬。组织学特征为:癌组织呈复杂的腺样结构或不规则的实性团巢状结构,癌细胞胞质丰富,均质状或颗粒状,嗜伊红,核大,核仁显著,部分核呈空泡状。特殊染色和免疫表型:4例均为dPAS、GCDFP-15、AR(+),ER、PR、CerbB-2、CK5/6、EGFR(-)。结论乳腺大汗腺癌罕见,但预后较好,诊断必须结合形态学及免疫组化染色结果。 Objective To explore the clinicopathologic features and prognosis of apocrine carcinoma of the breast.Methods Four case of apocrine carcinoma of the breast was studied with HE stains,dPAS stains and immunohistochemical stains and then review of the literature.Results This group of 4 cases were female,age at onset of 45~58 yeas old,average age of 51.5.Clinically characterized were painless and hard texture breast lumps.Apocrine carcinoma of the breast had special pathological changes.The cancer cells arranged into complex structure of the adenoid and irregular solid nest-like structure.The tumor was almost completely composed of apocrine cells;cells with abundant cytoplasm may be eosinophilic and vacuolated.Special staining show that the tumor cells were positive for dPAS;Immuno-histochemical stains show that the tumor cells were positive for GCDFP-15 and AR.Conclusion Apocrine carcinoma of the breast is a rare kind of breast tumors with a good prognosis.Its diagnosis mainly relies to morphology and immunohistochemical stains.
