摘要
目的探讨脑脊液中低分子量蛋白质Cystatin C和β2-MG在神经系统疾病中的与其它蛋白指标的关系和临床意义。方法观察多种神经系统疾病患者脑脊液中Cystatin C、β2-MG、C.S.F Total Potein(TP)和C.S.F MicroAlbumin(MAlb),根据临床需求部分患者检测了QAlb(血清白蛋白与脑脊液微量白蛋白比值,计算公式为SAlb/MAlb)。(1)根据β2-MG实际测定结果四分位分组,采用单因素方差分析(ANOVA),并对比分析组间Cystatin C、TP、MAlb及MAlb/TP值;(2)根据QAlb分组,采用单因素方差分析(ANOVA)并对比分析组间Cystatin C、β2-MG、TP及MAlb;(3)分析Cystatin C与β2-MG、TP、MAlb及QAlb的相关系数显著性检验及β2-MG与TP、MAlb及QAlb的相关系数显著性检验。结果 (1)根据β2-MG四分位分组,组间ANOVA结果显示:各组间Cystatin C、MAlb及TP比较均有统计学意义,而MAlbl/TP比值组间比较无统计学意义(p分别为0.00863、<2e-16、<2e-16及0.167);(2)根据QAlb分组,组间ANOVA分析结果显示:组间β2-MG、MAlb及TP比较均有统计学意义(P<2e-16),而组间Cystatin C比较无统计学意义(P>0.05);(3)Cystatin C与β2-MG呈正相关,且相关系数显著性检验有统计学意义(P<0.05);Cystatin C与TP、MAlb及QAlb均呈负相关,但其相关系数显著性检验无统计学意义(P>0.05);β2-MG与Cystatin C、TP、MAlb及QAlb相关系数显著性检验均有统计学意义(P<0.01)。结论脑脊液Cystatin C和β2-MG均为低分子量蛋白质,其分子量相当接近,但在神经系统疾病中的表达并不一致,提示此两种蛋白质在疾病中的价值及机制有所区别,联合观测Cystatin C及β2-MG并结合脑脊液中其它蛋白指标在神经系统疾病的判断、治疗与预后中可能具有重要价值,尚有待更进一步的临床观察及总结。
Objective To explore the clinical values and relations of the concentrations of lumbar CSF Cystatin C and β2-microglobulin (β2-MG)with other protein biomarkers. Methods The eerebrospinal fluid (CSF) samples were collected from patients with nervous system diseases(NS diseases) ,and the concentrations of Cystatin C,β2-MG and other parameters of protein in CSF(CSF MicroAlbumin(MAlb),total protein(TP),etc)were observed and detected, QAlb (Serum Albumin/MAlb)were observed and detected in part of CNS diseases; (1)Cystatin C, MAIb, TP and MAlb/TP were compared, according to β2-MG's interquartile ; (2) Cystatin C, β2-MG, MAlb and TP were compared,according to QAlb (3)and the correlation coefficient significant inspection of Cystatin C with β2-MG, MAlb,TP and Qa,b and the correlation coefficient significant inspection of β2-MG with MAlb,TP and QAlb were analyzed. Results (1) According to β2-MG's interquartile, statistical significance was observed in Cystatin C, MAlb, TP and MAlb/TP(p value= 0. 00863, 〈 2e-16 , 〈2e-16 , 0. 167) ;(2) According to QAlb, statistical significance was observed in O2-MG, MAlb and TP, but without statistical significance in Cystatin C; (3)the correlation of Cystatin C with 132-MG was 0.21(P=0. 003),and correlations of Cystatin C with TP, MAlb and QAlb were without statistical significance(P〉0.05) ;the correlations of 132-MG with TP, MAIb and QAlb were statistical significant(P〈0. 001). Conclusions There was difference in the levels of Cystatin C and β2-MG in CSF. Combination of detection of Cystatin C and β2-MG with other protein biomarkers has great importance for the diagnosis, treatement and prognosis of NS diseases,which requires further clinical observation and summary.
