摘要
目的探讨影响无剖宫产指征初产妇选择剖宫产的因素。方法采用质性研究中的现象学研究方法对10例无剖宫产指征选择剖宫产的初产妇进行半结构式、面对面的深度访谈,运用Colaizzi资料分析原则进行资料整理分析,提炼主题。结果无剖宫产指征初产妇选择剖宫产的影响因素集中于4个主题:心理因素;社会因素;自身因素;医源性因素。结论初产妇对自然分娩的恐惧和对剖宫产的信任是选择剖宫产的主要原因,故须加强产前教育,消除初产妇对分娩的恐惧、紧张的心理,使其树立信心,提倡人性化护理服务模式;同时,正确引导并帮助初产妇选择适宜的分娩方式,降低剖宫产率。
Objective To explore the factors influencing cesarean section selection for primiparae without cesarean indictors. Methods By phenomenological research methods of qualitative research, 10 primiparae without cesarean indictors underwent the semi-structured and face-to-face depth interviews. Colaizzi analysis principles were adopted to analyze the documents and to refine the themes. Results The influencing factors for cesarean section selection for primiparae without cesarean indictors focused on the four themes： psychological factors,social factors, their own factors and iatrogenic factors. Conclusion For primiparae, the fear of natural childbirth and the trust of cesarean section are the main reasons for choice of cesarean section, and thus nursing staff should enhance prenatal education, eliminate the early maternal fear of child- birth and the psychological tension, build up the confidence and promote the humane care service model. Moreover, proper guidance and help should be provided for primiparae to select the appropriate mode of delivery to reduce the cesarean section rate.
