摘要
目的探讨颈椎生理曲度随年龄的变化情况,为临床应用提供解剖学资料。方法选择年龄10~86岁的健康志愿者120例,按年龄段分为6组。在透视状态下摄标准颈椎侧位片,分别测量各年龄段C1~C7的垂直距离、C2~C7各椎体之间的距离、颈椎曲度值(Borden测量法)及弧弦距至第7颈椎后下缘垂直距离,并进行比较。结果随着年龄的增长,颈椎C1~C7的垂直距离出现先增大后减小的趋势,其中20~29、40~49岁年龄组颈椎C1~C7的垂直距离明显大于60~86岁年龄组(F=2.730,q=3.414~4.421,P<0.05)。不同年龄组间颈椎曲度值无明显差异(P>0.05)。60~86岁年龄组弧弦距至第7颈椎后下缘的垂直距离出现明显下移,与其他各组比较,差异有极显著性(F=3.533,q=4.481~5.270,P<0.01);而其他各年龄组间弧弦距至第7颈椎后下缘的垂直距离无明显差异(P>0.05)。60~86岁年龄组C5-6椎体之间的距离明显小于其他年龄组(F=3.526,q=2.964~5.146,P<0.01),其他各年龄组间C5-6椎体之间的距离无明显差异(P>0.05);各年龄组其他椎体之间的距离亦无明显差异(P>0.05)。结论颈椎生理曲度的变化主要表现为60岁以后C5-6椎间隙后缘距离呈锐减趋势,从而导致颈椎生理曲度的曲顶位置下移。
Objective To explore the changes of physiologic curvature of cervical vertebrae with age,and provide anatomic data for clinical reference.Methods This study consisted of 120 healthy volunteers-age 10-86 years-who were divided into six groups.The X-ray films of lateral projection of cervical vertebrae were taken.The C1-C7 vertical dimension,distance between two vertebral bodies of C2-C7,the values of cervical curvature and vertical dimension from arc-chord spur to the C7 of each age group were measured and compared.Results The C1-C7 vertical dimension showed the tendency of increase first and then decrease,in which,the vertical dimension in 20-29-and 40-49-year-old group was markedly greater than that of over-60-year-old group(F=2.730,q=3.414-4.421,P0.05).The cervical curvature among different-age groups was not significantly different(P0.05).The cervical curvature position of over-60-year-old group obviously moved down,compared with the other groups,the differences showed extremely significant(F=3.533,q=4.481-5.270,P0.01),and the differences of that among the other age groups were not significant(P0.05).The distance between C5 and C6 in over-60-year-old group was shorter than the other age groups(F=3.526,q=2.964-5.146,P0.01),and no significant differences were noted among the other age groups.Conclusion The main changes of physiologic curvature of cervical vertebrae mainly appears after 60 years of age,the tendency of rapid decrease of the distance between rear edge of C5 and C6 results in moving down the top position of cervical curvature.
