摘要
目的:探讨5种不同材料的全冠修复体对基牙龈沟液(GCF)中的天冬氨酸转氨酶(AST)和碱性磷酸酶(ALP)水平的影响。方法:选择拟行固定冠修复的病例50例,随机分为5组,分别采用镍铬合金、钴铬合金、纯钛、金合金、瓷全冠修复。在修复前、修复后1、3、6、12、24个月分别进行牙周临床检查并测定、分析龈沟液(GCF)量和GCF中AST、ALP的总量。结果:修复后1个月时,5组之间各项指标均无统计学差异(P>0.05)。修复后3、6、12、24个月时,镍铬合金组、钴铬合金烤瓷冠组各项指标均较修复前明显增加,且呈递增趋势(P<0.05)。而纯钛组、金合金组、全瓷组在修复后1、3、6、12、24个月各时间点与修复前相比各项指标均无明显改变,差异无统计学意义(P>0.05),且3组间各项指标同期相比亦无明显差异(P>0.05)。结论:临床制作合格的镍铬合金烤瓷冠、钴铬合金烤瓷冠对牙周组织有一定的不利影响;而纯钛烤瓷冠、金合金烤瓷冠、全瓷冠对牙周组织的影响相对较小。
AIM:To investigate the effects of five full-crown materials on the amount of gingival crevicular fluid(GCF),and on the level of aspartate aminotransferase(AST) and alkaline phosphatase(ALP) in GCF of abutment teeth.METHODS: The anterior teeth of 50 patients were randomly restored by Ni-Cr,Co-Cr,Gold alloy PFM,Ti PFM and all ceramic crowns.Before and 1,3,6,12,24 months after full-crown restoration,GCF was collected at the abutment teeth.The levels of AST and ALP were measured by an automated chemical analyzer.RESULTS:One month after restoration,no significant difference was found in all indices among the five groups(P〉0.05).GCF,AST and ALP time-dependently increased in Ni-Cr and Co-Cr alloy groups(P〈0.05).However,GCF,AST and ALP only increased slightly in Ti,Gold PFM and all ceramic crown groups(P〉0.05).CONCLUSION: Ti PFM,Gold PFM and all ceramic crowns may exert less influence on periodontal tissues than Ni-Cr and Co-Cr based PFM.
