摘要
目的探讨大剂量甲强龙治疗重度特发性面神经麻痹的疗效。方法 34例重度特发性面神经麻痹住院患者随机分为对照组和实验组,各17例。2组常规治疗相同,给予维生素B1100mg,甲钴胺500μg,肌内注射,1次/d;并辅以局部理疗。对照组给予静滴甲强龙80mg,1次/d,7d后停用,实验组给予静滴甲强龙120mg,1次/d,7d后停用。静脉给药停止后,改为口服强的松30mg,每周减5mg,直至减完停药。治疗后1个月和2个月时采用H-B面神经功能分级系统评分评价疗效。结果对照组和实验组治疗后1个月治愈率分别为29.41%、52.94%,治疗后2个月分别为58.52%、94.12%。总有效率治疗1个月分别为52.94%、88.24%;治疗2个月为76.47%、100%,2组差异有统计学意义(P<0.05)。结论大剂量甲强龙治疗重度特发性面神经麻痹的疗效优于常规剂量治疗。
Objective To investigate the effect of large dose methylprednisolone for severe idiopathic facial nerve paralysis. Methods Results Totally 34 patients with severe idiopathic facial palsy in hospitalized patients were randomly divided into ex- perimental group and control group, 17 cases in each group. Patients in the two groups were given vitamin B1 100 mg, and meeobalamin 500/,g intramuscular injection, once a day; supplemented by local physiotherapy. The control group was given in- travenous methylprednisolone 80 mg, once a day, for 7 days. The experiment group received intravenous methylprednisolone 120 rag, once a day, for 7 days; after cessation of intravenous administration, prednisone 30 mg oral, weekly reduction of 5 rag, until less finished withdrawal. After 1 month and 2 months of treatment, the H--B function of the facial nerve grading system score was used to evaluate the therapeutic effect. Conclusion After treatment of 1 month, the cure rates were respee-tive 29.41%, 52.94% in the control group and experimental group. After treatment of 2 months, the cure rates were respec- tive 58.52%, 94. 12%. The total efficiency were respective 52. 94%, 88. 24% after 1 month of treatment, and 76. 47%. 100% after 2 months of treatment, there was obvious difference between two groups （P〈0.05）. Conclusion Large dose meth- ylprednisolone in the treatment of severe idiopathic facial nerve paralysis is more effective than conventional dose therapy.
