摘要
传统的腹外疝修补术是一种有张力修补,且不符合解剖生理,故导致术后5年复发率高达10%~15%。而巴德(Bard)公司提供的Marlex材料是由聚丙烯单丝织成的圆锥型网塞及网片,近年来用于疝修补术,已受到病人和临床医师的喜爱和欢迎,因此种修补是一无张力修补,其材料不仅有强大的抗感染能力,与组织的相容性强,且承受腹腔内压力,使术后复发率明显下降。我院外科于1998年4月以来,先后对20例腹外疝病人用网塞及网片进行修补。其中腹股沟疝16例,腹壁切口疝3例,脐旁疝1例,均取得近期满意疗效。随访1~6月,无切口疼痛、尿潴留、复发等。切口均一期愈合。此法与传统的疝修补术相比,有手术时间短、创伤小、术后疼痛轻、下床活动早、恢复快、感染率低、并发症少等优点,更适宜于并发心肺功能不全、前列腺肥大的老年人及术后复发疝的病人。
Conventional hernia repair, with suture line tension, which is not physiologically reasonable, leads to a high 5-year recurrent rate from 10% to 15% .Marlex mesh and plug, provided by Bard Inc, has been widely used in hernioplasty in recent years. This polyprophylene prosthesis is strong, unable to harbor infection and has a high tissue compatibility. This repair is out of tension and generally accepted by surgeons and patients because of its low recurrent rate. In 1998, we used this mesh prosthesis in 20 cases of hemioplasty, in which 16 of inguinal hernia, 3 of abdominal incisional hernia, 1 of umbilical hernia. In the 1 to 6 months of follow-up, none of the following complications occurred, incisional infection, pain, bladder retention, liquid collection of scrotum. Compared with conventional repair, this technique has the advantages of short operative time, small injure, little postoperatively discomfortable, rapid recover and low infection. So it is suitable for those patients complicated with heart or pulmonary disfunction, hypertrophy of prostate gland or with high ages.
