摘要
目的观察高渗盐水鼻腔冲洗治疗儿童上气道咳嗽综合征的疗效。方法 395例患儿随机分成口服药物组和鼻腔冲洗组:口服药物组:132例,根据不同的病因给予口服开瑞坦、香菊颗粒及抗生素等常规治疗连续5d。鼻腔冲洗组:263例,除根据不同的病因给予口服香菊颗粒、开瑞坦及抗生素等常规治疗外,同时给予高渗盐水鼻腔冲洗,每日1次,连续治疗5d。根据临床表现判断疗效。结果口服药物组:132例,显效41例,(31.1%);好转67例,(50.7%);无效24例,(18.2%);总有效率81.8%。鼻腔冲洗组共263例,显效155例,(58.9%);好转103例(39.1%);无效5例,(0.2%);总有效率99.8%。两组疗效比较,P<0.005。鼻腔冲洗组疗效明显优于口服药物组。结论高渗盐水鼻腔冲洗具有冲洗鼻腔分泌物,减少鼻腔分泌,杀菌、消炎、减轻鼻腔黏膜水肿等功能,能够减轻不同原因引起的儿童上气道咳嗽综合征的症状,缩短病程及严重影响儿童学习、睡眠、进食等临床用药时间,无任何副作用,儿童依从性好,因此高渗盐水鼻腔冲洗治疗上气道咳嗽综合征是一种值得推广应用的好方法。
Objective To observe the curative effect of Child upper airway cough syndrome nasal douched by hypertonic saline solution. Methods 395 cases were randomly divided into the oral medication group and the nasal irrigation group： the oral medication group（n=132） were given the conventional treatment of the oral Clarityne, Chinese crossostepium leaf particles and antibiotics for five days according to different causes, the nasal irrigation group（n=263） were given the conventional treatment of the oral Clarityne, Chinese crossostepium leaf particles and antibiotics according to different causes, while giving hypertonic saline nasal irrigation once daily, five days of continuous treatment. We judge the efficacy based on clinical manifestations. Results The oral medication group, 132 cases were cured, excellent in 41 cases （31.1%）; improved in 67 cases （50.7%）; ineffective in 24 cases （18.2%）; the total effective rate of 81.8%. The nasal irrigation group, 263 cases were cured, excellent in 155 cases （58.9%）; improved in 103 cases （39.1%）; ineffective in 5 cases （0.2%）; the total effective rate of 99.8%. Comparison of curative effect of two groups, P〈0.005. The nasal irrigation group were significantly better than the oral medication group. Conclusion Hypertonic saline nasal irrigation has the functions of flushing the nasal secretions, decreasing of nasal secretion, sterilization, anti-inflammatory, relieving nasal mucosal edema, reducing the symptoms caused by different causes of child upper airway cough syndrome, shorten the course of disease and serious impact on children＇s learning, sleep, eating and other clinical medication time, without any side effects, children with good compliance. Therefore hypertonic saline nasal irrigation for treatment of child upper airway cough syndrome is a good method worthy of popularization and application.
