摘要
目的探讨盐酸戊乙奎醚(长托宁)联合阿托品治疗急性重度有机磷中毒(SAOPP)的临床疗效及不良反应。方法 180例SAOPP患者随机分为阿托品组(I组)、长托宁组(Ⅱ组)及长托宁+阿托品组(Ⅲ组),观察3组中毒症状消失时间、胆碱酯酶(CHE)活力恢复时间、平均住院时间、治愈率、药物中毒率、药物反弹率、中间综合征发生率及其他不良反应。结果Ⅲ组中毒症状消失时间为(7.3±5.8)h,Ⅱ组为(8.5±6.6)h,Ⅰ组为(20.5±11.0)h;Ⅲ组CHE活力恢复时间为(10.2±9.8)h,Ⅱ组为(12.3±10.8)h,Ⅰ组为(28.7±18.3)h;Ⅲ组平均住院时间为(8.4±1.4)h,Ⅱ组为(8.8±1.5)h,Ⅰ组为(11.2±5.3)h;Ⅲ组中间综合征发生率为9.84%,Ⅱ组为14.54%,Ⅰ组为31.25%;Ⅲ组治愈率为96.72%,Ⅱ组为87.27%,Ⅰ组为76.56%;Ⅲ组不良反应少,与Ⅰ组比较,差异具有统计学意义(P<0.05);Ⅲ与Ⅱ组的差异无统计学意义(P>0.05)。结论长托宁联合阿托品治疗SAOPP疗效显著,可缩短病程,提高治愈率,减少不良反应,是目前治疗SAOPP有效的方法,值得临床使用。
[Objective]To investigate the clinical effect and adverse reactions of penehyclidine hydrochloride（long-acting tuoning） combined with atropine in the treatment of acute severe organophosphorus poisoning（SAOPP）.[Methods]180 SAOPP patients were randomly divided into the atropine group（group I）,the long-acting tuoning group（groupⅡ）,and the long-acting tuoning combined with atropine group（groupⅢ）.The disappearing time of poisoning symptoms,the recovery time of cholinesterase（CHE）activity,the average hospital stay,the cure rate,the rate of drug poisoning,the medication rebound rate and the incidence rate of intermediate syndrome of three groups were observed.[Results]The disappearing time of poisoning symptoms of group Ⅲ,groupⅡ and group I was respectively（7.3±5.8） h,（8.5±6.6） h and（20.5±11.0） h.The recovery time of CHE activity of group Ⅲ,groupⅡ and group I was respectively（10.2±9.8） h,（12.3±10.8） h and（28.7±18.3） h.The average hospital stay of group Ⅲ,groupⅡ and group I was respectively（8.4±1.4） h,（8.8±1.5） h and（11.2±5.3） h.The incidence rate of intermediate syndrome of group Ⅲ,groupⅡ and group I was respectively 9.84%,14.54% and 31.25%.The cure rate of group Ⅲ,groupⅡ and group I was respectively 96.72%,87.27% and 76.56%.The incidence rate of adverse reactions in group Ⅲ was significantly lower than that in group I（P0.05）.[Conclusion]The long-acting tuoning combined with atropine in treatment of SAOPP has obvious curative effect,and can shorten the course of the disease,improve the cure rate,reduce the incidence rate of adverse reactions.It is the effective therapy for SAOPP,and is worthy of clinical application.
