实时三维超声心动图评价PCI疗效的研究 被引量:7

Quantitative Assessment of Clinical Value of PCI by Real-Time Three-Dimensional Echocardiography

摘要 目的探讨RT-3DE及其定量技术在评价心脏PCI短期疗效中的临床应用价值。方法对冠脉造影提示其冠状动脉至少一支有明显狭窄(>70%)的患者36例行PCI治疗,在术前、术后1周和术后3个月行RT-3DE检查,应用Qlab分析软件,得到左室l6节段时间-容积曲线,记录左室舒张末容积(EDV)、左室收缩末容积(ESV)、左室射血分数(LVEF),计算出16节段最小收缩容积时间标准差(Tmsv 16-SD)和最大差异(Tmsv 16-Dif)。结果与PCI术前相比,术后1周所有患者的心率无明显变化,LVEF值无明显升高(P>0.05),Tmsv 16-Dif、Tmsv 16-SD均显著减少(P均<0.05)。然而术后1周、术后3个月分别与术前相比较,Tmsv 16-SD、Tmsv 16-Dif、Tmsv16-SD(%)、Tmsv16-Dif(%)均有所减小,有统计学意义(P<0.05)。结论 PCI可明显改善冠心病患者左室心肌运动的同步性及其收缩功能。RT-3DE得出的左室l6节段时间-容积曲线形态及参数能够为PCI治疗的短期疗效评估提供较完整的定量信息。 Objective To explore the quantitative assessment of the short-term efficacy and clinical value of PCI by real-time three-dimensional echocardiography(RT-3DE). Methods Thirty six patients with coronary artery disease (CAD) and having at least one stenosis more than 70 percent at a vessel coronary artery underwent PCI. All patients before and 7 d and 3 months after PCI were examined by RT-3 DE, respectively. Full-volume RT-3DE data was acquired. The LV 16-segmental time-volume curves were obtained by the QLAB software. Then 16 regional LV volume-time curves were drawn, and the end-diastolic volume( EDV), end-systolic volume(ESV) and ejection fraction(EF) were extracted from the curves. The remarkably difference in standard deviation(SD) and maximal difference(Dif) of the time to point with minimal systolic volume(Tmsv) of 16 segments were recorded. Results Compared the data at a week after PCI with those before PCI, there were no differences in heart rate and LVEF(P 〉 0.05) ,but both Tmsv 16-Dif and Tmsv 16-SD decreased(P 〈 0.05). A week and three months after PCI, Tmsv16-SD,Tmsv16- Dif,Tmsv16-SD( % ) and Tmsvl6-Dif( % ) all decreased as compared to that before PCI(P 〈 0.05 ). Conclusion PCI can improve the synchronization of left ventricular myocardial motion and systolic function. The shape of left ventricular 16 segmental time-volume curves and parameters obtained by RT-3 DE can provide more complete quantitative information for the PCI' s shortterm efficacy of the treatment.
