针刀微创治疗脑瘫肢体畸形的技术建立及临床应用 被引量:8

摘要 目的提出脑瘫肢体畸形病理机制及治疗规范。方法根据针刀微创具有解除痉挛、挛缩的功效,建立了切割纠畸术、肌肉刺激术、神经触激术(脊神经触激术与周围神经触激术)三种操作技术。切割纠畸术:切割挛缩的肌组织或切断痉挛肌的部分肌纤维,以减弱肌肉或者肌腱的单元力量,促进痉挛肌和拮抗肌平衡;肌肉刺激术:在痉挛肌肉的肌腹部,对肌肉强刺激,以增加肌肉收缩和舒张的频率,从而抑制异常姿势反射和运动模式,消除或减轻痉挛;神经触激术:对神经给予适度的触激,促进神经兴奋或超限抑制,消除或减轻肌痉挛或改善肌张力。结果运用自身前后对照的研究方法来进行临床疗效的观察,近愈率分别是:马蹄足畸形92%,膝关节屈曲畸形和足内、外翻畸形84%;腕掌屈、背伸畸形80%,膝过伸畸形47%。结论提出了脑瘫肢体畸形治疗的新理念,建立及规范了针刀微创操作技术,促进了行业科技进步。 Objective To propose the pathological mechanism and the treatment specification of limb deformities in cerebral palsy. Methods Based on the function of acuptomy minimally invasive therapy as relieving spasm and eontracture, three operation techniques were established, named incision technique, muscle stimulation technique and nerve trigger technique( spinal nerve trigger technique and peripheral nerve trigger technique). Incision technique:the muscular tissue was incised or a part of muscular fibers of spastic muscle was cut off so as to weaken the unit strength of muscle or tendon and promote the balance of spastic muscle and antagonistic muscle. Muscle stimulation technique:a strong stimulation was applied to the spastic muscle belly so as to increase the frequency of muscle contraction and dilation,inhibit abnormal postural re flex and motion pattern and eliminate or alleviate spasm. Nerve trigger technique:a proper triggering was ap plied to the nerve so as to promote nerve excitation or prohibitive inhibition, eliminate or alleviate muscular spasm or improve muscular tension. Results With the self comparison before and after treatment, the clini cal efficacy was observed. The remarkably effective rates were 92% for equines foot deformity ,84% for flex ion deformity of knee joint, strephenopodia and strephexopodia,80% for the deformities of palmar flexion and dorsiflexion and 47% for knee hyperextention deformity. Conclusion The new idea is proposed in the treat ment of limb deformities in cerebral palsy. The techniques of acuptomy minimally invasive manipulation are established and standardized. The industrial scientific technological progression is promoted.
