摘要
目的探讨动力外固定架结合克氏针治疗C型桡骨远端骨折的临床效果。方法回顾性分析2005年1月至2012年2月采用动力外固定架结合克氏针治疗C型桡骨远端骨折的病例。本组46例,男26例,女20例,年龄25～65岁,平均年龄54.5岁;按AO/OTA分型:C1型26例,C2型17例,C3型3例。比较本组术前术后影像学指标,功能指标,并采用Green-o'Brien的腕关节评分标准对术后疗效进行评估。结果所有患者均获得随访,随访时间6-12个月,平均10.2个月。X线片显示骨折全部愈合。按Green-o'Brien的腕关节评分标准:优31例,良10例,可5例,差0例。本组钉道感染3例,固定松动2例,腕关节僵硬3例。结论动力外固定架结合克氏针治疗C型桡骨远端骨折可达到较为满意的效果,临床可根据患者的具体情况选择治疗方式。
Objective To investigate the clinical effects of dynamic external fixator combined with K-wire in the treatment of type C distal radius fractures according to the association for the study of internal fxation （AO）. Methods From January 2005 to February 2012, 46 cases of AO type C distal radial fractures which had been treated by dynamic external fixator combined with K-wire were retrospectively analyzed. In this group there were 26 males and 20 females, with an average age of 54.5 years old （range; 25-65 years）. According to AO/Orthopedic Trauma Association （OTA） classification, there were 26 cases of type C 1, 17 of type C2, and 3 of type C3. The postoperative radiographic parameters and function parameters were compared with that before surgery, and the curative effects were assessed by Green-o＇Brien functional score. Results All patients were followed up for an average period of 10.2 months （range; 6-12 months）. The X-ray films showed all fractures were healed. The clinical outcomes were evaluated according to Green-o＇Brien functional score. There were 31 patients reaching excellent results, 10 good, 5 fair and 0 poor. There were 3 cases of pin tract infection, 2 of fixator loosening, and 3 of wrist stiffness. Conclusions Dynamic external fixator combined with K-wire in the treatment of the AO type C distal radial fractures can achieve satisfactory clinical results. The selection of treatment methods depends on the patients＇ clinical condition.
