摘要
目的探讨百白破疫苗引起过敏的临床表现及诊治经验,以提高认识,更好地预防和治疗疾病。方法报道百白破疫苗过敏致双手发红肿胀瘙痒36天1例,并结合国内近10年来己报道的百白破疫苗引起异常反应与偶合症病例进行文献复习。结果百白破疫苗异常反应和偶合症临床表现愈来愈多样,而且过敏反应消失时间与偶合症的数量有增加趋势。10年间,己报道因百白破疫苗而发生异常反应367例,偶合症19例;异常反应中以局部化脓最多见,有262例,其次到紫癜66例、神经性水肿15例、过敏性休克7例、死亡7例、惊厥3例、荨麻疹3例、癫痫2例、剥脱性皮炎1例、反复高热1例;其中反应后治愈的时间10天以内的有287例,占78.2%;超过10天的有38例,占10.35%;超过20天的有42例,占11.45%;偶合症中以上呼吸道感染与和意外死亡多见,各占26.32%。结论在进行百白破疫苗注射时要严格掌握适应症,避免疾病的潜伏期、发病期、恢复期进行疫苗注射,注射后要叮嘱家长注意观察患儿的不良反应,如出现要及早就医;医务人员对产生过敏的患儿早期合理使用足够的药物,能加速患儿的康复。
Objective To analyze and summarize clinical manifestations and treatment experience of DTP vaccine allergies. Methods Data of one case with DTP vaccine caused hands redden swelling itching 36 days, and reviewed DTP vaccine about abnormal reaction and coupled disease patients reported in china in the past 10 years. Results Clinical manifestation of DTP vaccine which was about abnormal reaction and coupled disease were more and more diverse, the recovery time of allergic reaction and the a- mount of coupled disease were increasing. 367 cases of abnormal reaction and 19 cases of coupled disease had reported in the past 10 years. Local putrefying, 262 cases, saw most in the abnormal reaction; Secondly, purpura 66 cases, neuropathic edema 15 cases, death 7 cases, anaphylactic shock 7 cases, convulsions 3 cases, .urticaria 3 cases, epilepsy 2 cases, exfoliative dermatitis 1 case, repeatedly high fever one case. Recovery time within 10 days had 287 cases, accounting for 78.2%. More than 10 days for 38 cases, accounting for 10.35% ; More than 20 days for 42 cases, accounting for 11.45%. The upper respiratory tract infection and death saw most in the couple disease, each accounted for 26.32 %. Conclusions DTP vaccination should strictly master the indications, avoid the disease of incubation, the period of disease and recovery. To Urge parents to watch the children of adverse effects after injection, early intervention and reasonable therapy after diagnosis, would accelerate children rehabilitation.
