实时三维超声心动图评价冠心病室壁运动幅度及舒张功能 被引量:15

Assessment of Ventricular Wall Motion State and Diastolic Function by Real-Time Three-Dimensional Echocardiography in Patients With Coronary Artery Disease

摘要 目的:应用实时三维超声心动图(RT-3DE)评价冠心病左心室室壁运动异常节段的位移及舒张功能。方法:35例健康查体者为正常对照组;75例冠心病患者为冠心病组,经冠状动脉造影证实前降支、右冠状动脉、回旋支病变,分为50%<狭窄≤70%亚组(n=62)和狭窄>70%亚组(n=73)。所有研究对象均行RT-3DE检查,应用QLAB定量分析软件,测算动脉相对应区域左心室壁运动异常节段的平均位移(平均位移)、平均充盈率,评价动脉狭窄相对应区域平均位移、平均充盈率与狭窄程度的相关性。结果:①冠心病组两亚组前降支、右冠状动脉、回旋支所支配区平均位移、平均充盈率均比正常对照组低,差异均有统计学意义(P<0.01);狭窄>70%亚组平均位移及平均充盈率均比50%<狭窄≤70%亚组低,差异均有统计学意义(P<0.01)。②冠心病患者动脉狭窄所支配区域室壁运动幅度改变与动脉狭窄程度呈负相关(r=-0.816,P<0.01);冠心病患者动脉狭窄所支配区域平均充盈率改变与动脉狭窄程度呈负相关(r=-0.708,P<0.01);冠心病组左室壁运动幅度改变与平均充盈率变化呈正相关(r=0.678,P<0.01)。结论:冠心病组动脉狭窄相应支配区域平均位移及平均充盈率明显减低,与动脉狭窄程度呈负相关。冠心病患者室壁运动幅度减弱与平均充盈率下降改变呈正相关。 Objective:To assess the wall motion and diastolic function of left ventricle in real-time three-dimensional echocardiography (RT-3DE) in patients with coronary artery disease (CAD). Methods : Our work included two groups, Normal control group, n = 35, and CAD group, n = 75, that with two subgroups of 50% 〈stenosis≤70 in 62 cases and stenosis〉70% in 73 cases. All the lesions were confirmed by coronary angiography. RT-3DE was performed in both groups, Q-lab software was used to calculate the average wall motion and mean filling rate (MFR). The re- lationship between average motion and MFR in regions of stenotic artery wall and severity of stenosis were evaluated. Resuhs:①The average wall motion and MFR in regions of stenotic artery were lower in CHD group than those in Control group,P〈0. 01. Average motion and MFR in stenosis 〉70% subgroup were lower than those in 50% 〈 stenosis ≤70 subgroup, P〈0. 01. ②In CHD group, the average motion and MFR were both negatively related to the severity of stenosis (r =-0. 816, P〈0. 01 ) and ( r= -0. 708 ,P〈0. 01 ) respectively,while average motion was positively related to MFR ( r-0. 678 ,P〈0. 01 ). Conclusion :The average wall motion and MFR in regions of stenotic artery in CHD patients were significantly lower than nor- real subjects. The average wall motion was positively related to MFR in CAD patients.
