摘要
[背景]观察环磷腺苷葡甲胺(MCA)联合芪苈强心胶囊治疗充血性心力衰竭(CHF)的疗效及血流动力学变化.[病例报告]选择心功能Ⅲ,Ⅳ级的CHF患者63例,随机分为3个组,每组各为21例;Ⅰ组给予常规治疗;Ⅱ组给予常规治疗+MCA;Ⅲ组给予常规治疗+MCA+芪苈强心胶囊.Ⅰ组总有效率为66.7%,Ⅱ组为80.1%,Ⅲ组为90.5%,Ⅲ组总有效率明显优于Ⅰ组(P<0.05).3组治疗后每搏输出量、心输出量、心脏作功指数及左心室射血分数均升高,其中Ⅱ,Ⅲ组升高显著(P<0.01),且与Ⅱ组比较,Ⅲ组明显升高(P<0.05,P<0.01);治疗后Ⅱ,Ⅲ组二尖瓣血流舒张早期最大流动速度与心房收缩期最大流动速度的比值与Ⅰ组比较均显著升高(P<0.01),且与Ⅱ组比较,Ⅲ组明显升高(P<0.05).3组治疗后BNP质量浓度均降低,但以Ⅱ,Ⅲ组与Ⅰ组比较降低显著(P<0.01),且与Ⅱ组比较,Ⅲ组又明显降低(P<0.05).[讨论]MCA联合芪苈强心胶囊治疗CHF有效、安全,两者具有协同作用.
BACKGROUND To investigate the curative effects and hemodynamic changes of meglumine cyclic adenylate(MCA) combined with the Qiliqiangxin capsule for the congestive heart failure(CHF).CASE REPORTS 63 patients with congestive heart failure with Ⅲ and Ⅳ grade of heart function were randomly divided into three groups(n=21),groupⅠ of conventional therapy,group Ⅱ of conventional therapy plus MCA and group Ⅲ of conventional therapy plus MCA combined with the Qiliqiangxin capsule.Total curative effective rate was higher in group Ⅱ(80.1%) and group Ⅲ(90.5%) than in group Ⅰ(66.7%),but total curative effective rate was obviously higher in group Ⅲ than in groupⅠ(P〈0.05).The stroke volume,cardiac output,cardiac index and left ventricle ejection fraction increased in three groups after treatment,especially increased significant in groupⅡ and Ⅲ(P〈0.01),and increased significantly higher in group Ⅱ than in group Ⅲ(P〈0.05,P〈0.01).The ratios of peak flow rate of bicuspid blood current diastolic phase to atrial systole period more increased in group Ⅱ and group Ⅲ than in group Ⅰ(P〈0.01),in group Ⅱthan in group Ⅲ(P〈0.05).The BNP concentrations decreased in three groups after treatment,especially decreased significant in group Ⅱ(P〈0.01) and group Ⅲ(P〈0.01) and decreased significant in groupⅡ than in group Ⅲ(P〈0.01).DISCUSSION MCA combined with tha Qiliqiangxin capsule has utility,safety and synergistic effects in treatment of CHF.
