摘要
目的探讨术中头位偏斜和眼球旋转导致散光轴位变化对常规准分子激光原位角膜磨镶术(laser in situ keratomileusis,LASIK)手术效果的影响。方法临床病人自身对照研究。对在解放军180医院眼科准备行LASIK的患者,经详细检查选取无眼部疾病、术前矫正视力1.0、双眼等效球镜度数差别〈-1.00D、双眼散光均〉-1.50D的患者30例,采用自身对照法:1只眼坐位视远时标记角膜水平和垂直轴位、术中调整,1只眼按常规卧位标记,对两眼术后6个月的视力、残余屈光度数和对比敏感度(contrast sensitivity function,CSF)进行配对t检验。结果两眼术后在夜眩光下3.0c/d(t=2.268,P=0.031)和夜眩光下6.0c/d(t=2.649,P=0.013)2种空间频率的CSF差异有统计学意义。结论对于〉-1.50D的散光患者,术中头位偏斜和卧位眼球旋转造成激光实际切削散光轴位与坐位视远时验光所确定散光轴位的误差会影响术后视觉质量,最好在坐位视远时标记角膜水平和垂直轴位,术中调整,提高术后视觉质量。
Objective To research the effect of astigmatic axes' changes by deviation of head and cyclotorsion in laser in situ keratomileusis (LASIK). Methods Thirty patients without ocular diseases whose binocular astigmatic degrees were greater than -1.50D and difference between binocular spherical equivalents were smaller than -1.00D were included. All eyes' best corrected vision were better than 1.0. Each case was chosen one eye into experiment group (marked horizontal and vertical axis in seated position and regulated in surgery) and the other eye into control group at random. The visual acuity, astigmatic degrees and contrast sensitivity function (CSF) of both eyes were analyzed by Paired-samples t test 6 months after LASIK. Results CSF of 3.0c ·d^-1 (t=2.268, P =0.031) and 6.0 c.d^-1 (t=-2.649, P =0.013) spatial frequency at night with glare revealed a statistically difference between both eyes. Conclusions Astigmatic axes' changes by deviation of head and cyclotorsion affect the visual quality in eyes which astigmatic degrees were greater than -1.50D after LASIK.
