原发性高血压患者血浆肾素活性水平与血压和尿钠的关系 被引量:9

摘要 目的分析原发性高血压患者血浆肾素活性水平与血压、尿钠排泄的关系。方法原发性高血压患者479例,根据血浆肾素活性水平的5分位排序,分为3组,低肾素组93例,中间肾素组291例,高肾素组95例。各组血浆肾素活性与24 h动态血压及24 h尿钠排泄情况进行相关性分析。结果与低肾素组比较,中间肾素组和高肾素组年龄明显偏低,血浆肾素活性明显增高,差异有统计学意义(P<0.05,P<0.01)。低肾素组老年人、女性、非杓型血压比例明显高于高肾素组,差异有统计学意义(P<0.01)。血浆肾素活性水平与24 h尿钠排泄、24 h血压呈负相关(P<0.05,P<0.01)。结论老年和女性原发性高血压患者血浆肾素活性水平较低。低肾素患者血压昼夜节律改变更明显,以非杓型血压多见。 Objective To study the relation of plasma renin activity level with blood pressure and natriuresis in essential hypertension patients. Methods Four hundred and seventy-nine essential hypertension patients were divided into low renin level group(n=93),medium rennin level group (n = 291), and high rennin level group (n = 95) according to their renin activity level. Relation of plasma rennin activity level with 24 h ambulatory blood pressure and natriuresis was analyzed in 3 groups. Results The age was slightly younger while the plasma renin activity level was signifi- cantly higher in medium and high renin level groups than in low renin level group(P^0.05, P~ 0.01)" The proportion of aged subjects, females, and non-dipper blood pressure was significantly higher in low renin level group than in high renin level group(P〈0.01). The 24 h plasma renin activity level was negatively related with the 24 h natriuresis and blood pressure(P〈0.05, P〈 0.01). Conclusion The plasma renin activity level is lower in aged subjects and females with es- sential hypertension. The change in circadian rhythm of blood pressure, especially non-dipper blood pressure,is more significant in patients with a low renin level.
