过夜配戴角膜塑型镜后角膜曲率及角膜厚度的变化 被引量:15

摘要 目的 :了解过夜配戴角膜塑型镜 (Orthokeratology ,简称OK镜 )后早期角膜曲率、角膜厚度及屈光度的变化。方法 :14例 2 5眼中低度近视患者 (平均屈光度 - 3.89± 1.2 6D) ,随机分成二组分别配戴Dk值不同的角膜塑型镜 :BE角膜塑型镜组 (7例 12眼 ,Dk≈ 140 ) ;E&E角膜塑型镜组 (7例 13眼 ,Dk≈ 5 8)。采用夜戴方式 ,平均配戴时间 10 .76± 1.2 7小时。用Orbscan角膜地形图系统检查配戴一晚后角膜曲率、角膜厚度的改变。同时记录裸眼视力及屈光度的改变。结果 :配戴一晚后所有患者的中央及周边的角膜均增厚 (P <0 .0 5 )。Orbscan角膜地形图Simk值在垂直轴及水平轴均变平 (垂直轴△Simk :0 .94± 0 .5 6D ,P <0 .0 0 1;水平轴△Simk :0 .996± 0 .6 8D ,P <0 .0 0 1)。裸眼视力平均提高 0 .4± 0 .2 4(P <0 .0 0 1)。屈光度平均减少 - 1.85± 0 .82D(P <0 .0 0 1)。Dk值不同的二组之间中央及颞上方位点角膜增厚程度的差别有统计学意义。结论 :过夜配戴角膜塑型镜后减低近视屈光度的效果明显。初次过夜配戴角膜塑型镜可导致角膜厚度轻度增加。 Objective:To assess early changes in response to ovenight wear of orthokeratology.Methods:Two kinds of orthokeratology lenses with different Dk value were fitted to 25 eyes of 14 patients(average refraction:-3.89±1.26D).The patients were randomly divided into two groups by the lenses Dk values: One group with BE orthokeratology lenses(7 patients, 12 eyes, Dk≈140);The other group with E&E orthokeratology lenses(7 patients, 13 eyes,Dk=58),All eyes were free o f pathology. All patients wear lenses ovenight. The average wear time was 10.76±1.27 hours. Changes in the radius and thickness of the cornea were measured with Orbscan corneal topography system after 1 night of lens wear. And unaided acuity and refractive changes were recorded.Results:Significant increases were observed both in central and peripheral cornea thickness(P<0.01).Simulated keratometry (Simk) reading indicated significant flattening in both the horizontal(△Simk:0.94±0.56D,P<0.001)and vertical meridians(△Simk:0.996±0.68D,P<0.001).The unaided acuity increased significantly by 0.4±0.24. Refraction significant decreases-1.85±0.82D. The difference in the increases of cornea thickness on central and superotemporal corneal locations between 2 groups has statistically significance.Conclusion:This study showed that orthokeratology is effective on reducing the refraction of myopia after overnight wear. And the cornea thickness increases slightly after 1 night wear. [FK(WB80011。5]
