不同心室率控制水平心房颤动患者血清高敏C反应蛋白的变化 被引量:1

The Investigation of Relationship between High-Sensitivity C-reactive Protein and the Level of Heart Rate in Patients with Atrial Fibrillation

摘要 目的通过检测房颤及不同心室率控制水平的房颤患者血清高敏C反应蛋白(hsCRP)水平与左心房(LA)直径,评价控制房颤患者心室率对于降低血清hsCRP,减少心脑血管并发症的临床意义。方法选取非阵发性心房颤动患者97人,依据静息心室率控制情况分为三组:房颤1组(静息心室率60次/分~79次/分);房颤2组(静息心室率80次/分~109次/分);房颤3组(静息心室率大于等于110次/分)。选取同期窦性心律的健康体检人群38人作为对照组。测定四组人群血清hsCRP和LA直径。结果与对照组相比,房颤组hsCRP和LA直径均增高,差异有统计学意义。房颤3组hsCRP及LA直径较房颤1组、房颤2组升高(P值分别为0.027,0.046,0.035,0.041),房颤1组与房颤2组两组间hsCRP及LA直径无统计学差异。线性相关分析显示血清hsCRP与LA直径、心率相关(相关系数分别为0.179、0.350,P值分别为0.026、0.022)。结论血清hsCRP及左心房直径在房颤及心室率控制欠佳的房颤患者中明显增高,控制房颤患者心室率能减轻心肌重构、降低hsCRP水平。 Objective To investigate the relationship between high-sensitivity C-reactive protein(hsCRP),left atrial internal diameter and level of ventricular rate in patients with atrial fibrillation(AF),and to assess the role of decreasing hsCRP by controlling the ventricular rate.Methods According to the level of resting ventricular rate,97 patients with AF were divided into three groups:group 1(resting ventricular rate from 60 bpm to 79 bpm),group 2(resting ventricular rate from 80 bpm to 109 bpm),group 3(resting ventricular rate ≥110 bpm),and 38 healthy people were studied as control.The hs-CRP and left atrial internal diameter were measured.Results HsCRP and left atrial internal diameter significantly increased in patients of AF than that in control group(P 0.05).The same result was seen in group 3 than that in group 1 and group 2.No difference of hsCRP was found between group 1 and group 2.Linear regression analysis showed a significant positive correlation between hsCRP and left atrial internal diameter,heart rate.Conclusions Serum hsCRP and left atrial internal diameter are related to the controlling degree of ventricular rate in patients with AF,which were reduced by controlling ventricular rate.
