CANTILEVERED RESIN BONDED FIXED PARTIAL DENTURES: A SYSTEM REVIEW

摘要 目的采用系统评价的方法研究单端树脂粘结固定桥的存留率和成功率。方法在Pubmed上以"denture,partial,fixed,resin-bonded"为检索词,检索1990年1月1日—2010年10月10日期间发表的文章,按照纳入与排除标准筛选文献、提取数据后,采用Stata 10.1软件进行Meta分析。结果在501篇论文中最终筛选出13篇论文(12项临床研究),共计930个修复体。针对不同实验设计类型进行的亚组分析结果显示,单端树脂粘结固定桥存留率是95%(95%CI 92%~98%),成功率是90%(95%CI 86%~94%);上颌修复体的单端树脂粘结固定桥成功率(89%)低于下颌(93%),差异有显著性(P=0.042);前牙区修复体的成功率(89%)低于后牙区(96%),但两者的差异无统计学意义(P=0.145)。其他影响因素无法建立有效的回归模型。结论上颌单端树脂粘结固定桥的成功率低于下颌;建议以后对单端树脂粘结固定桥进行高质量、大范围、长期的临床随机对照实验并提供详细的临床数据。 Objective To systematically evaluate survival rate and success rate of cantilevered resin bonded fixed partial dentures (CRBFPDs). Methods A search in Pubmed for medical subjects with heading terms of "denture, partial, fixed, resinbonded ", publishrd between Janury 1, 1990 and October 10, 2010. The literatures were screened and collected according to the in- clusion and exclusion criteria, and the data were analyzed by using the Stata 10.1 software. Results Thirteen articles (12 cli- nical studies) with a total of 930 prosthesis were finally selected form 501 articles. The results of subgroup analysis according to different experiment designs showed that the overall survival rate was 94% (95 % CI 92%- 97%), and the success rate was 90 (95%CI 86%- 93%). The success rate of maxillary cantilevered CRBFPDs (89%) was lower than that of mandibular ones (93%), the difference was statistically significant (P=0.042). The success rate for cantilevered CRBFPDs placed on posterior teeth (96%) tended to be higher than that for anterior-placed CRBFPDs (89%), the difference between them was not statistically significat (P=0.145). It was impossible to build a reliable regression model by other influencing factors. Conclusion The success rate of maxillary cantilevered CRBFPDs is lower than that of mandibular ones; more high-quality, large scale, long-terra ran- domized controlled clinical trials are suggested.
