显微镜下鼓室成型与听骨链重建术在临床上的应用效果对比

摘要 目的探讨显微镜下鼓室成型与听骨链重建术在慢性化脓性中耳炎治疗中的应用效果,为临床治疗中耳炎提供参考。方法选取80例(80耳)慢性化脓性中耳炎手术患者分为治疗组与对照组各40例,对照组采用显微镜下鼓室成型方法,治疗组采用显微镜下听骨链重建术。结果两组患者四个频率(500 Hz、1 000 Hz、2 000 Hz、4 000 Hz)的骨导均值经过治疗后都有明显下降,但是治疗组的下降幅度更大,与对照组治疗后的值对比有明显差异(P<0.05)。所有患者在术前后气骨导差均有明显下降。治疗组较对照组在气骨导差缩小更加明显(P<0.05)。结论相对于显微镜下鼓室成型方法,显微镜下听骨链重建术治疗慢性化脓性中耳炎能从多方面有效改善多听阈的听力状况,值得推广应用。 Objective To investigate the effects of attic molding and ossicular chain reconstruction for treating chronic suppurative otitis media in microscope for giving a reference for the clinical treatment of otitis media. Methods 80 patients (80 ears) with chronic otitis media were equally divided into the treatment group and the control group, each group had 40 cases. The treatment group was treated by ossicular chain reconstruction, and the control group was treated by attic molding. Results The average bone conduction in the four frequency (500 Hz, 1 000 Hz, 2 000 Hz, 4 000 Hz) of two groups were decreased significantly after treatment, but the treatment group decreased more than that of the control group (P 〈 0.05). All patients in the air-bone gap were significantly reduced after surgery, and the treatment group were narrowed more than that of the control group (P 〈0.05). Conclusion Compared to the attic molding method, ossicular chain reconstruction in the treatment of chronic suppurative otitis media can improve hearing thresholds of hearing status in the microscope, should be widely applied.
