摘要
分析妊娠期急性胰腺炎(acute pancreatitis,AP)患者发病的临床特点,总结护理要点,完善护理措施,提高母婴的生存质量。文中总结收治的23例妊娠期AP患者的临床资料,发病时平均孕周32±5(28-37)周。23例孕妇均痊愈,10例保胎成功,自然分娩健康儿;8例在控制胰腺炎的情况下行剖宫产术,分娩健康儿;4例因胎窘而胎死宫内,行清宫术,1例因胎膜早破行引产术,产出死胎。妊娠早期并发胰腺炎的治疗方法与非妊娠期胰腺炎相同,积极维护和治疗原发病,同时加强对母亲、胎儿的病情观察,及时发现异常病情,必要时在保全母亲安全的同时及时终止妊娠。加强妊娠期AP(尤其是重症急性胰腺炎)患者的监护,可提高母婴成活率。
To analyze the clinical characteristics and management of pregnancy complicated with acute pancreatitis, evaluate the main points of nursing and improve the patients＇living quality. A retrospective study was carried out on 23 cases of gestational patients complicated with acute pancreatitis admitted in our center from 1999 to 2010. All the patients were cured. Among them, 12 ca- ses were natural childbirth, 7 cases were treated by cesarean operation after acute pancreatitis improved, and the other 4 cases were fetal death. At the early stage of pregnancy complicated with acute pancreatitis, the therapeutic measures were similar to the non-preg- nant acute pancreatitis. Besides conservative treatment of acute pancreatitis, consecutive monitoring of the pregnant patient and fetus was also essential, and if acute pancreatitis were considered dangerous in gestational period, termination of pregnancy should be performed as soon as possible. Taking intensive care of pregnancy complicated with acute pancreatitis, especially severe acute pancreatitis, was the preferred treatment of the disease.
