摘要
目的观察急性期细菌性脑膜炎(BM)和病毒性脑炎(VEM)患儿血清和脑脊液中可溶性协同刺激分子B7-H3(CD276)、IL-1β的水平变化,探讨其临床意义。方法收集2010年7月-2011年11月在本院神经内科住院的BM患儿17例、VEM患儿12例的急性期脑脊液和血清。采用双抗体酶联免疫吸附法(ELISA)检测各组对象血清和脑脊液B7-H3和IL-1β水平,应用SPSS 17.0统计软件对各组进行分析,并对B7-H3和IL-1β进行相关性分析。结果 BM组患儿脑脊液中B7-H3与IL-1β水平均显著高于VEM组(P<0.01,0.05);BM组患儿血清中B7-H3水平明显高于VEM组(P<0.05)。BM组和VEM组血清IL-1β的变化无统计学差异(P>0.05)。BM组脑脊液中B7-H3与IL-1β水平变化呈正相关(r=0.526,P<0.05)。结论协同刺激分子B7-H3在脑脊液和血清中的水平测定具有急性期BM和VEM的鉴别诊断价值;B7-H3与IL-1β水平可以作为BM的早期诊断的参考指标之一。协同刺激分子B7-H3与炎症因子IL-1β参与了细菌性脑膜炎的急性期炎症反应过程。
Objective To detect the content of soluble costimulatory molecule B7-H3(CD276) and interleukin-1β(IL-1β) in cerebrospinal fluid(CSF) and serum in children with bacterial meningitis(BM) and viral encephalitis(VEM),and to explore their clinical application value. Methods The acute phase of CSF and serum were collected from 17 cases of BM children and 12 cases of VEM children,who were admitted into the Department of Neurology of Children′s Hospital Affiliated to Soochow University from Jul.2010 to Nov.2011.Enzyme linked immunosorbent assay(ELISA) method was adopted to determine the levels of B7-H3 and IL-1β in CSF and serum in BM and VEM.The data were analyzed by using SPSS 17.0 software,and the correlation analysis between the level of B7-H3 and IL-1β was processed. Results The levels of B7-H3 and IL-1β in CSF of BM were significantly higher than those of VEM(P〈0.01,0.05);The levels of B7-H3 in serum of BM was higher than that of VEM(P〈0.05).The levels of IL-1β in serum of BM and VEM were not of significant difference(P〉0.05).The levels of B7-H3 and IL-1β in CSF of BM were positively correlated(r=0.526,P〈0.05). Conclusions Co-stimulatory molecule B7-H3 levels in CSF and serum can be one of differential diagnosis targets between BM and VEM at their early stage.The levels of B7-H3 and IL-1β in CSF can be one of the early diagnosis indexes of BM.Co-stimulatory molecule B7-H3 and inflammatory factor IL-1β probably take part in the inflammatory process of BM at acute phage.
