摘要
目的探讨尿微量清蛋白(UMA)与糖尿病足(DF)之间的关系。方法选取我院2007—2010年内分泌科住院糖尿病患者526例,其中UMA(+)患者103例,UMA(-)患者423例。比较UMA(+)与UMA(-)患者DF患病率;UMA(+)非糖尿病足(NDF)患者与UMA(-)NDF患者随访1年,观察新发DF病例数;将UMA(+)NDF患者分为UMA>200 mg/L患者与20 mg/L<UMA≤200 mg/L患者,随访1年观察新发DF病例数。结果 UMA(+)患者DF患病率(33.98%)显著高于UMA(-)患者(3.78%)(P<0.01)。UMA(+)NDF患者68例,UMA(-)NDF患者407例,随访1年,前者DF发病率(35.29%)显著高于后者(9.58%)(P<0.01)。68例UMA(+)NDF患者中,UMA>200 mg/L患者25例,20 mg/L<UMA≤200 mg/L患者43例,随访1年,前者DF发病率(56.00%)显著高于后者(23.26%)(P<0.01)。结论 UMA与DF的发生有关,且UMA水平越高,DF发病率越高。故早期干预UMA有助于预防DF的发生。
Objective To explore the relationship between urine minimum albumin （UMA） and diabetic foot （DF）. Methods 526 patients admitted to the department of endocrinology of our hospital were selected. Of all the patients, there were 103 UMA （ ＋ ） patients and 423 UMA （ - ） patients. The incidence of DF between UMA （ ＋ ） patients and UMA （ - ） pa- tients was compared. UMA （ ＋ ） non - diabetic foot （NDF） patients and UMA （ - ） NDF patients were followed up for one year to observe new DF cases. UMA （ ＋ ） NDF patients were divided into UMA 〉200 mg/L group and 20 mg/L 〈 UMA≤200 mg/L group, and then they were followed up for one year to observe new DF cases. Results DF incidence of UMA （ ＋ ） patients （ 33.98% ） was significantly higher than that of UMA （ - ） patients （3. 78% ） （ P 〈 0. 01 ） . The 68 cases of UMA （ ＋ ） NDF and 407 cases of UMA （ - ） NDF were followedup for one year, the incidence of DF in UMA （ ＋ ） NDF patients （35.29%） was significantly higher than that in the UMA （ - ） NDF patients （9.58%） （ P 〈0. 01 ） . Among the 68 UMA （ ＋ ） NDF pa- tients, 25 cases were UMA 〉200 mg/L and 43 cases were 20 mg/L 〈 UMA≤200 mg/L After one year following up, the inci- dence of DF of the former was 56. 00%, significantly higher than that of the latter （23.26%） （P 〈 0. 01 ）. Conclusion The occurrence of DF is related to UMA, with the incidence of DF getting higher when the UMA level becomes higher. Early interven- tion of UMA is conducive to the prevention of DF.
