软通道微创与内科保守治疗高血压脑出血的近期与远期临床疗效比较 被引量:14

Short and Long Term Effect Comparison of Minimally Invasive Surgery with Soft Tube and the Internal Medical Expectant Treatment in Hypertensive Intra-cerebral Hemorrhage

摘要 目的:比较软通道微创与内科保守治疗高血压脑出血的近期与远期临床疗效。方法:将86例高血压脑出血患者随机分入对照组与观察组,给予40例对照组患者内科保守治疗,给予46例观察组患者软通道微创治疗。比较两组住院期间病死率、再出血率及出院后1年日常生活能力。结果:观察组与对照组患者住院期间病死率分别为19.6%和27.5%(P<0.05),观察组显著低于对照组(P<0.05);两组再出血率分别为23.9%和25.0%,差别无统计学意义(P>0.05);治疗后1年观察组日常生活能力评定优良率为65.7%,显著高于对照组41.4%,差别具有统计学意义(P<0.01)。结论:软通道微创治疗高血压脑出血可显著减少住院期间病死率,改善患者预后,提高生活质量。 Objective: To compare the short and long term effect of minimally invasive surgery with soft tube and the internal medical expectant treatments in hypertensive intra-cerebral hemorrhage.Method: 86 cases with hypertensive intra-cerebral hemorrhage were randomly divided into control group and experment group.40 patients of control group received internal medical expectant treatment and 46 cases of experiment group were given microinvasive surgery with soft tube treatment.The mortality and recurrent rehemorrhage rate during in-hospital period and ADL after treatment were compared between two groups.Result: The mortality during in-hospital period were 19.6% and 27.5% in experience group and control group respectively(P0.05);Recurrent rehemorrhage rate during in-hospital period were 23.9 and 25.0% in two groups without significant difference(P0.05);ADL excellent and good rate 1 year after treatment were 65.7% and 41.4% in experience group and control group respectively(P0.01).Conclusion: Microinvasive surgery with soft tube for hypertensive intra-cerebral hemorrhage can greatly decrease the mortality during in-hospital period,improve the patients prognosis and life quantify.
