认知康复训练对急性脑梗死后认知功能障碍患者脑电特征的影响 被引量:22

摘要 目的观察认知康复训练对急性脑梗死后认知功能障碍患者脑电特征的影响。方法将急性脑梗死后认知障碍患者63例按随机数字表法分治疗组(n=32)和对照组(n=31)。2组患者均进行相同的药物治疗和常规康复训练,治疗组患者在药物治疗和常规康复训练的基础上增加认知康复训练。入组当天(治疗前)和治疗4周后(治疗后)采用简易精神状态量表(MMSE)、简式Fugl—Meyer运动功能评定量表(FMA)分别评定2组患者的认知功能、运动功能,采用脑电图分析脑电特征与MMSE和FMA评分的相关性。结果治疗后,治疗组认知功能改善总有效率为84.4%,与对照组的总有效率58.1%比较,差异有统计学意义(P〈0.05)。治疗后,治疗组患者的FMA评分为(95.12±12.33)分,与组内治疗前的(82.04±10.01)分和对照组治疗后的(88.03±11.51)分比较,差异均有统计学意义(P〈0.05)。相关性分析显示,脑电图相关通道β段相对功率改变与MMSE评分呈正相关(r=0.879,P〈0.01),与FMA评分无相关性(r=-0.232,P〉0.05)。结论药物加常规康复训练联合认知康复训练对急性脑梗死后认知障碍患者认知和运动功能有较好的疗效,脑电特征可作为评价认知功能的替代或重要的辅助工具。
